Tunica vaginalis Free Grafts for Closure of Urethrocutaneous Fistulas

In 32 patients a tunica vaginalis free graft was used for closure of urethrocutaneous fistulas (n = 26) or complicated hypospadias repair (n = 6). All patients had at least one operation (average 3.9) for urethral reconstruction prior to this procedure. 12 patients had recurrent fistulas (average 2.2). The technique proved to be very successful. Only 2 patients had to undergo subsequent reoperation for recurrent fistulas. The use of a free tunica vaginalis graft interposed between skin and urethra is our method of choice in the repair of recurrent or complicated urethrocutaneous fistulas.

Long-term followup of buccal mucosa onlay graft for hypospadias repair: analysis of complications.

We review the long-term results of buccal mucosa onlay grafting for urethral reconstruction in hypospadias surgery in patients with followup of at least 5 years.We retrospectively evaluated 132 patients who underwent buccal mucosa onlay graft for hypospadias repair, including 34 who underwent "salvage" grafting, during a 10-year period at our institution. In 49 cases with available followup longer than 5 years (average 6.2) complications were analyzed in detail.The overall complication rate was 24% (12 of 49 patients), with all but 3 complications occurring during the first postoperative year (3 fistulas, 1 stricture, 1 meatal stenosis, 2 graft contractures, 2 scars at oral harvesting site)…

Lingual mucosal graft urethroplasty for anterior urethral reconstruction.

Objective: Evaluate the use of lingual mucosal graft (LMG) in anterior urethral strictures.Methods: From January 2001 to December 2006, 29 men (mean age, 48.5 yr) with anterior urethral strictures underwent graft urethroplasty with LMG. The mean length of stricture was 3.6 cm. Patients with bulbar, penile, or bulbopenile strictures received one-stage dorsal free graft urethroplasties. In patients with failed hypospadias repair we performed a two-stage urethroplasty. Criteria for successful reconstruction were spontaneous voiding with no postvoid residual urine and no postoperative instrumentation of any kind. A low-pressure reservoir for urine is created by antimesenteric splitting and side-to-side anastomosis of the rectosigmoid, the expectation being to obtain better continence rates and better protection of the upper tract than are achievable by ureterosigmoidostomy. Between 1990 and August 1993 the procedure was performed in 73 patients (59 adults and 14 children) whose mean age was 43.5 years. The indications were malignancy (n = 55), bladder exstrophy/epispadias (n = 14), trauma (n = 3), and sinus urogenitalis (n = 1). Standardized knowledge about genital function in adult female individuals with exstrophy-epispadias complex (EEC) is scarce. Carp’s-tongue swords - morphological, metallurgical and cultural aspects

This contribution discusses the results from a study of the spatial distribution of different morphological sub-groups of carp’s-tongue swords at a European scale, relating morphological types to the metallurgial make-up of the respective pieces. From this study some surprising conclusions concerning the chronological as well as the spatial dimension of the evolution of carp’s-tongue swords emerge. We also look at the degree of variability in the deposition of these artefacts and in the composition of the assemblages in question, with considerable repercussions for our understanding of the depositional contexts of metal objects in the Atlantic Bronze Age world in general.

Abstract Evidence from recent research combined with an evaluation of the literature indicates that Arisaema is adapted to pollination by fungus gnats. It apparently shares this peculiarity among aroids only with the distantly related genus Arisarum . In addition to previous records from Japan and North America, systematic collections from nine Arisaema species during several expeditions in the Himalayas in Nepal showed that, although other less efficient insect groups may participate, the nematoceran families Mycetophilidae and Sciaridae are the principal pollen vectors; they best fit the pollination apparatus of the mainly (para)dioecious kettle trap blossoms.
