Material deprivation affects the management and clinical outcome of hepatocellular carcinoma in a high-resource environment
2021
Abstract Aim This study investigated how material deprivation in Italy influences the stage of hepatocellular carcinoma (HCC) at diagnosis and the chance of cure. Methods 4114 patients from the Italian Liver Cancer database consecutively diagnosed with HCC between January 2008 and December 2018 were analysed about severe material deprivation (SMD) rate tertiles of the region of birth and region of managing hospitals, according to the European Statistics on Income and Living Conditions. The main outcomes were HCC diagnosis modalities (during or outside surveillance), treatment adoption and overall survival. Results In more deprived regions, HCC was more frequently diagnosed during surveillan…
Recent Advances in the Treatment of Patients with Multiple Myeloma
2020
Simple Summary The evolving data from trials assessing novel combinations as a part of the frontline and relapse treatment in transplant and non-transplant candidates have markedly improved the anti-myeloma efficacy of the different therapeutic regimens and improved patients’ prognosis. Current treatment objectives are focused to further improve the rate of complete remission, time to progression, progression-free survival and overall survival without increasing toxicity. Besides, different strategies are being developed in the elderly population as this group of patients requires a closer monitoring with individualized, dose-modified regimens to improve tolerability while maintaining their…
Cancer and Pregnancy: A Comprehensive Review
2021
Simple Summary The co-occurrence of cancer and pregnancy is fortunately a rare event. It is a challenging situation for all involved parties, as the risks and benefits of oncological treatments should be balanced for both the mother and the offspring. This work offers guidance to clinicians regarding the choice of diagnostic and therapeutic procedures, as well as regarding the management of the pregnancy. Abstract Cancer diagnosis and treatment in pregnant women is a challenging situation. A multidisciplinary network of specialists is required to guide both, the expecting mother and the unborn child through the diagnostic workup and the cytotoxic therapy, by balancing the respective risks a…
Risk factors for recurrence after robot-assisted radical hysterectomy for early-stage cervical cancer: a multicenter retrospective study
2020
This retrospective analysis aimed to assess the risk factors for recurrence in patients diagnosed with early-stage cervical cancer (&le
A Multicentric Randomized Trial to Evaluate the ROle of Uterine MANipulator on Laparoscopic/Robotic HYsterectomy for the Treatment of Early-Stage End…
2021
ObjectiveThis prospective randomized trial aimed to assess the impact of the uterine manipulator in terms of lymph vascular space invasion (LVSI) in patients undergoing minimally invasive staging for early-stage endometrial cancer.MethodsIn this multicentric randomized trial, enrolled patients were randomly allocated in two groups according to the no use (arm A) or the use (arm B) of the uterine manipulator. Inclusion criteria were G1-G2 early-stage endometrial cancer at preoperative evaluation. The variables collected included baseline demographic characteristics, perioperative data, final pathology report, adjuvant treatment, and follow-up.ResultsIn the study, 154 patients (76 in arm A an…
Renal haemodynamics and severity of carotid atherosclerosis in hypertensive patients with and without impaired renal function.
2014
Background and Aim. Renal resistance index (RRI), assessed by Duplex-Doppler sonography, has been classically considered as a mere expression of intrarenal vascular resistance. Recent studies, however, have showed that RRI is also influenced by upstream factors, especially arterial compliance, confirming its possible role as a marker of systemic vascular alterations. Several studies have shown that carotid intima-media thickness (cIMT) and carotid plaques (cP), assessed by ultrasonography, are documented markers of subclinical organ damage as well as expression of progressive atherosclerotic disease, and that they get worse with the progressive deterioration of renal function. The study was…
Spatio-temporal pattern of cells expressing the clock genes period and timeless and the lineages of period expressing neurons in the embryonic CNS of…
2010
The initial steps towards the generation of cell diversity in the central nervous system of the fruitfly Drosophila melanogaster take place during early phases of embryonic development when a stereotypic population of neural progenitor cells (neuroblasts and midline precursors) is formed in a precise spatial and temporal pattern, and subsequently expresses a particular sequence of genes. The clarification of the positional, temporal and molecular features of the individual progenitor cells in the nerve cord and brain as well as of their specific types of neuronal and/or glial progeny cells forms an essential basis to understand the mechanisms controlling their development. The present study…
Experimental Study of Central Baffle Flume
2019
In this paper, the hydraulics of a central baffle flume (CBF) were experimentally studied. Extensive experimental runs were carried out to highlight the effects of different geometrical parameters such as baffle length, contraction ratio, and the length of upstream and downstream guide walls. Two different approaches, i.e., the Π theorem of dimensional analysis coupled with the incomplete self-similarity theory and the energy balance equation, were applied to deduce the stage–discharge formula of a CBF characterized by different geometrical parameters. The stage–discharge relationship deduced by the dimensional analysis approach was more accurate than that obtained by the energy approach. F…
One shoot seldinger central venous catheterization in dialyzed patients
2015
Introduction: Central Venous Catheterization is necessary in uremic patient (before dialysis) and many other conditions. In this study we demonstrated the advantages of ultrasonography to perform the procedure. Materials and methods: 48 uremic patient were submitted to ultrasound-guided central venous catheterization. The procedure was performed following the Seldinger “one shot” technique. Results: The mean operative time was 4 minutes, with a high rate of success (100%) and a low percentage of complications (2%). Conclusion: The ultrasound-guided central venous catheterization is a safe procedure, rapid and easy to perform. The procedure has a low rate of failures and complications and a …
Oxygenation of Cervix Cancers: Impact of Clinical and Pathological Parameters
2003
Current knowledge on the oxygenation status of carcinomas of the uterine cervix is mostly based on pretherapeutic data obtained in pre-and postmenopausal women. Mean and median 02 tensions (pO2) obtained from >20,000 p02-measurements in more than 400 squamous cell carcinomas of the uterine cervix observed by several research groups were, on average, distinctly lower than in normal tissues. Oxygen tensions measured in the normal cervix of nullipara patients revealed a median pO2 of 42 mmHg, whereas in conscious patients with locally advanced cancers of the cervix (stage FIGO lb-IVa), the median pO2 values were 10-20 mmHg. In patients under general propofol-anesthesia the median p02 values we…
