Prehospital diagnosis in stroke patients-differentiation between cerebral ischemic events and cerebral hemorrhage

A large number of stroke patients is treated by the Mainz-EMS during the first hours after the onset of symptoms. A combination of anamnestic and clinical variables included in the score prehospitally leads to the tentative diagnosis confirmed by CT-scan in the majority of cases. It should therefore be possible in the future to initiate specific therapy for stroke patients prehospitally.

Age is known to affect prefrontal brain structure and executive functioning in healthy older adults, patients with neurodegenerative conditions and TBI. Yet, no studies appear to have systematically investigated the effect of age on cognitive performance in patients with focal lesions. High rate of cardiac thrombus diagnosed by adding cardiac imaging in acute stroke computed tomography protocol

Background Detection of left atrial appendage thrombus (LAAt) in acute stroke patients can be improved by cardiac computed tomography using prospective electrocardiogram-gated volume acquisition, which was added to the acute stroke computed tomography protocol in our institution in 2018. Aims To evaluate the factors and clinical management associated with LAAt in patients with acute ischemic stroke. We investigated the cognitive effects of microbleeds and other neuroimaging markers for cerebrovascular disease in a large, hospital-based study. Reliability and Accuracy of Echocardiography for Follow-up Studies after Intervention

By two-dimensional echocardiography limitations of M-mode echocardiography have been eliminated. As a direct and noninvasive method two-dimensional echocardiography seems to be an ideal method for analysis of left ventricular function in follow-up studies. In relation to other methods there are no limitations concerning x-ray exposure, physicians and patients safety. Before different studies can be discussed, reliability and accuracy of two-dimensional echocardiography have to be evaluated. Beat-to-beat, day-to-day, intra- and interobserver variability will be discussed, followed by description of follow-up studies after intervention.
