Surgical and anatomical basis of autonomic nerve-preserving mesorectal excision for rectal cancer

Adequate knowledge of the anatomy of the pelvis is a prerequisite to carry out a correct surgical technique in resection of rectal cancer. In this way, we can perform radical surgery, preserve the autonomic nerves and decrease local recurrence and urinary and sexual morbidity. The key to all this and the difference in results depends on a meticulous and detailed surgical technique.

Simulation for training in sinus floor elevation: new surgical bench model

Objectives: to describe a bench model (workshop of abilities) for sinus floor elevation (SFE) training that simulates the surgical environment and to assess its effectiveness in terms of trainees’ perception. Clinical Scenario: Unexpected Positive Nodes at Radical Cystectomy

Positive lymph nodes are found in 20–30 % of patients undergoing radical cystectomy for bladder cancer. Their presence is associated with poor survival in the majority. Super-extended lymph node dissection is recommended in the presence of positive lymph nodes to possibly improve oncologic outcome. In patients with unsuspicious lymph nodes, dissection should be performed at least up to the common iliac vessels. Even in the presence of lymph node metastases cystectomy is indicated to reduce local tumor complications. The type of urinary diversion should not be altered by lymph node positivity unless the diversion would otherwise negatively influence the extent of the procedure.
