Kinematics of shoulder abduction in the scapular plane

In this paper the kinematics of arm abduction in the scapular plane of 38 healthy experimental subjects is reported. The ratio of the glenohumoral and the scapulothoracic components of the motion were determined and the influence of the abduction speed and of the external load on it were investigated. The investigation of the effect of abduction speed involved all 38 subjects. Each of them performed one slow and one fast abduction. Statistical analysis showed that there were large differences between individuals, but that for any one individual the abduction process is essentially reproducible. Ultrasound observation of tissue fluid infiltration causing stridor in a woman undergoing shoulder arthroscopy

Respiratory stridor developed rapidly during an interscalene brachial plexus block for shoulder arthroscopy in an obese woman. Extensive cervicothoracic edema due to tissue diffusion of the arthroscopic fluid was suspected. The outcome of patient after conservative management was satisfactory. We give a brief review of how this complication develops, the ultrasound findings, and briefly discuss similar cases.

The radiation exposure during biomechanical investigation of the shoulder joint done with a digital radiography system (Polytron 1000 VR - Siemens) was measured experimentally. Total radiation dose and surface dose were determined by phantom measurements. The radiation exposure per image is about 1/6 to 1/7 of that of a conventional study. The digital radiography system used is thus suitable for kinesiologic studies of large joints in healthy volunteers.

Optimal positioning of the humeral component in the reverse shoulder prosthesis.

Total reverse shoulder arthroplasty is becoming more and more the standard therapeutic practice for glenohumeral arthropathy with massive lesions of the rotator cuff. The biomechanical principle of this prosthesis is represented by the reversion of the normal anatomy of the shoulder joint. This non-anatomical prosthesis leads to a medialization of the rotation centre of the glenohumeral joint and also to a distalization of the humeral head. All that causes a deltoid tension increasing so allowing a larger abduction of the arm. MR-Tomographie versus CT-Arthrographie bei glenohumeralen Instabilitäten

In a prospective study the diagnostic value of MRI compared to CT arthrography (CT-A) was evaluated in 26 patients with 27 instable shoulder joints. Surgical and/or arthroscopic correlation was available in all cases. Both methods showed a high accuracy (96% CT-A, 94% MRI) in the evaluation of the glenoid labrum. CT-A was significantly superior to MRI in the detection of capsular lesions (sensitivity 96% vs. 44%, accuracy 96% vs. 72%, negative predictive value 96% vs. 67%). 24 patients up to two weeks after primary traumatic shoulder dislocation were examined at 0.5 and 1.5 T. Surgical and/or arthroscopic correlation was available in 13, CT-arthrographic correlation in 16 patients. A joint effusion allowing sufficient evaluation of the capsulolabral complex was present in 21/24 (87.5%) cases. 11/14 patients with combined dislocated detachments of the glenoid labrum and capsular lesions were subsequently operated upon.
