We report on a rare homozygous intragenic deletion encompassing exons 1-6 of the SMN1 gene in a patient with spinal muscular atrophy (SMA) born into a consanguineous family. This exceptional configuration induced misinterpretation of the molecular defect involved in this patient, who was first reported as having a classic SMN1 exon 7 deletion. This case points out the possible pitfalls in molecular diagnosis of SMA in affected patients and their relatives: exploration of the SMN1 exon 7 (c.840C/T alleles) may be disturbed by several non-pathological or pathological variants around the SMN1 exon 7. The gamma-aminobutyric acid (GABA) antagonist (+/-)-bicuculline inhibits specific [3H]strychnine binding to postsynaptic glycine receptor sites in rat spinal cord synaptosomal membranes with an inhibition constant of about 5 microM, which is fairly similar to its inhibition constant reported for the GABA receptor. This effect is highly stereospecific, since the affinity of (-)-bicuculline for the specific [3H]strychnine binding sites is more than ten times less than that of the pharmacologically active (+)-bicuculline. Mit Hilfe des Lungenpraparates von Dijstra und Noyons und der Auswertungsmethode von Corsten4 wurden im Liquor von 32 Geisteskranken die Anwesenheit und die Menge Acetylcholin bestimmt. In 33 % der Falle war kein Acetylcholin nachweisbar. Bei Katatonen fand sich haufiger Acetylcholin. Es ist moglich, das es Beziehungen zwischen Krankheit und Acetylcholingehalt des Liquors gibt, aber es sind weitere Untersuchungen erforderlich, um zu sicheren Schlussen zu kommen. In einigen Fallen bewirkt der Liquor eine Erschlaffung der Lungen an Stelle einer Kontraktion. Vielleicht hangt das mit der Anwesenheit anderer bisher unbekannter Stoffe zusammen.

Objetivos: Determinar el efecto de un programa de entrenamiento de pedaleo de brazos (PB) estacionario, sobre el sistema cardiorrespiratorio de personas con tetraplejia. Metodo: Se estudiaron 11 participantes con tetraplejia por lesion medular (LM) de origen traumatico a niveles entre C4 y C6. Se midieron variables espirometricas (VC, FVC y MVV) y de Variabilidad de la Frecuencia Cardiaca VFC (HR, STDHR, RMSSD, LF, HF y HRVPOWER), antes y despues de 8 semanas de entrenamiento. Verletzungen der Wirbelsaule sind auch heute noch eine grose Herausforderung fur die behandelnden Unfallchirurgen.Vom Konzept der uberwiegend konservativen Behandlung ist man im letzten Jahrhundert infolge einer differenzierteren Diagnostik gerade bei den sog. instabilen Verletzungen abgekommen. Diskrepanzen bestehen nach wie vor in der Stabilitatsbeurteilung. In den letzten Jahren haben sich sowohl dorsale als auch ventrale oder kombinierte Versorgungen etabliert. Verengte Spinalkanale und neurologische Defizite sind dabei wichtige Faktoren. Anhand der Literatur werden die verschiedenen Vorgehensweisen und Beurteilungen diskutiert und abschliesend unserer eigenes Konzept vorgestellt.

OBJECTIVE AND IMPORTANCE: Epidural hematoma (EDH) of the spine represents an uncommon neurosurgical disorder that sometimes requ;res emergent surgical decompressive therapy. Traumatic EDH of the cervical spine is exceedingly rare. The hematoma is usually located dorsally in the epidural space. We present one case of acute EDH located ventrally in the cervical spine. Special emphasis is placed on the role of spinal endoscopy in surgical treatment.
