Feasibility and utility of a cognitive screening for risk stratification in hospitalized older patients

2017

OBJECTIVES To determine whether the Mini-Cog can be applied by nursing staff to hospitalized elderly patients for cognitive impairment associated risk stratification. METHODS This explorative prospective multicenter cohort study was carried out among 2522 patients aged 70 and older, hospitalized due to physical illness in eight hospitals in Rhineland-Palatinate, Germany. All patients were asked to conduct the Mini-Cog at the day of admission and were clustered into low-performance, intermediate-performance, and good-performance categories by trained nursing staff and two experienced geronto-psychiatrists as gold standard. Complications in the course of the treatment were monitored. RESULTS …

D-dimer for risk stratification in haemodynamically stable patients with acute pulmonary embolism

2015

Patients with submassive pulmonary embolism (PE) have a higher short-term mortality than those with low-risk PE. Rapid identification of submassive PE is important for adequate treatment of non-massive PE. We aimed to investigate the utility of D-dimer for the prediction of submassive PE stadium in normotensive PE patients.Normotensive PE patients were classified into submassive or low-risk PE groups. In addition to the comparison of the groups, area under the curve (AUC) and D-dimer cut-off for the prediction of submassive PE stadium, multi-variate logistic regression for association between D-dimer values above this cut-off and submassive PE stadium were also calculated. The data of 129 no…

The COVID-19 Assessment for Survival at Admission (CASA) Index: A 12 Months Observational Study

2021

Objective: Coronavirus disease 2019 (COVID-19) is a disease with a high rate of progression to critical illness. However, the stratification of patients at risk of mortality is not well defined. In this study, we aimed to define a mortality risk index to allocate patients to the appropriate intensity of care. Methods: This is a 12 months observational longitudinal study designed to develop and validate a pragmatic mortality risk score to stratify COVID-19 patients aged ≥18 years and admitted to hospital between March 2020 and March 2021. Main outcome was in-hospital mortality. Results: 244 patients were included in the study (mortality rate 29.9%). The Covid-19 Assessment for Survival at Admi…

Analysis of four years of ceilometer-derived aerosol backscatter profiles in a coastal site of the western Mediterranean

2018

Abstract We present the analysis of four years of measurements by a CL51 ceilometer in Burjassot (39.51 N, 0.42 W), a research station in the western Mediterranean coast. The 1-min resolution profiles of the CL51 are corrected, calibrated, grouped and cloud-screened to create a 1-h resolution database of aerosol backscatter profiles at 910 nm (βa) spanning from July 2013 to August 2017. A total of 21,247 βa profiles are obtained, covering 58% of the considered period. The analysis of the βa profiles as a function of the main aerosol in the atmosphere reveals that dust arrives at Burjassot mainly in the form of elevated layers, with a highest impact between 1 and 4 km. The βa profiles obtain…

Opportunistic Germination Behaviour of Gypsophila (Caryophyllaceae) in Two Priority Habitats from Semi-arid Mediterranean Steppes

2011

Gypsophila tomentosa and G. struthium are closely related species, characteristic of two European priority habitats, salt and gypsum inland steppes, respectively. Germination strategies of the two taxa were investigated in plants from two nearby populations, growing under the same climatic conditions but on different types of soil, and belonging to different plant communities. Their germination patterns were studied at five constant temperatures in darkness: 5oC, 10oC, 15oC, 20oC and 25oC, and the base temperature and the thermal time requirement were calculated. As the distribution area of both species is subjected to a Mediterranean continental climate with significant differences between…

Fine stratification of microbial communities through a metagenomic profile of the photic zone

2017

ABSTRACTMost marine metagenomic studies of the marine photic zone analyze only samples taken at one or two depths. However, when the water column is stratified, physicochemical parameters change dramatically over relatively short depth intervals. We sampled the photic water column every 15m depth at a single point of an off-shore Mediterranean site during a period of strong stratification (early autumn) to evaluate the effects of small depth increases on the microbiome. Using genomic assembly and metagenomic read recruitment, we found major shifts in the community structure over small variations of depth, with most microbes showing a distribution limited to layers approximately 30 meters th…

Food plant diversity as broad-scale determinant of avian frugivore richness.

2007

The causes of variation in animal species richness at large spatial scales are intensively debated. Here, we examine whether the diversity of food plants, contemporary climate and energy, or habitat heterogeneity determine species richness patterns of avian frugivores across sub-Saharan Africa. Path models indicate that species richness of Ficus (their fruits being one of the major food resources for frugivores in the tropics) has the strongest direct effect on richness of avian frugivores, whereas the influences of variables related to water–energy and habitat heterogeneity are mainly indirect. The importance of Ficus richness for richness of avian frugivores diminishes with decreasing sp…

A semi-empirical approach for predicting two-phase flow discharge through branches of various orientations connected to a horizontal main pipe

2010

Abstract The subdivision of two-phase flow in branching conduits consisting of a large horizontal main pipe with upward, downward, or lateral branches of reduced diameter is of great interest in various technological fields. For example, these conduits are important in light-water nuclear reactors (LWRs) in the case of a small break loss-of-coolant accident (SBLOCA) in a leg of the reactor's primary coolant loops, as well as for breaks or valve malfunctions in a large pipeline. In these kinds of circumstances, the relevant phenomenology often involves phase stratification coupled with possible liquid entrainment or gas pool-through phenomena. Therefore, these phenomena were studied in depth…

Impact of the EndoPredict-clin score on risk stratification in ER-positive, HER2-negative breast cancer after considering clinical guidelines.

2012

542 Background: Many ER-positive, HER2-negative breast cancer patients are treated by adjuvant chemotherapy according to current clinical guidelines. We retrospectively assessed whether the combined gene expression/ clinicopathological EndoPredict-clin (EPclin) score improved the accuracy of risk classification in addition to considering clinical guidelines. Methods: Three clinical breast cancer guidelines (National Comprehensive Cancer Center Network (NCCN), German S3 and St. Gallen 2011), and the EPclin score - assessed by quantitative RT-PCR in formalin-fixed paraffin-embedded tissue - were used to assign risk groups in 1,702 ER-positive, HER2-negative breast cancer patients from two ra…
