A questionable diagnosis in a woman with drowsiness

A 60-year-old caucasian woman was admitted to our Division of Internal Medicine for a 10-day history of drowsiness, irritability, dysphasia, weakness and difficulties in the maintenance of an upright position with numerous falls, with a traumatic lumbar spine injury and on the left shoulder. Then, while general clinical conditions were getting worse with drowsiness and hyposthenia of the right side of the body, after neurological evaluation, she was recovered in hospital. Pathological history includes a 20-year epilepsy and depression with emotional lability, behavior’s disorders and attempted suicide. Introduction: Acquired thrombotic thrombocytopenic purpura (aTTP) is a rare, but life-threatening, hematological disorder characterized by severe thrombocytopenia, hemolytic microangiopathic anemia, and frequent organ damage. The underlying pathophysiology of aTTP is a functional deficiency of plasma ADAMTS13 activity caused by antibodies directed against the ADAMTS13 protease. Despite plasma exchange (PEX) and immunosuppression with corticosteroids, and, more recently, rituximab, which achieve remission in most patients with aTTP, 10-20% of patients are refractory to treatment and die as a result of disease progression. Der Pulmonalarteriendruck des Menschen bei Kreislaufver�nderungen im Phenothiazin-Schlaf

An 11 kreislaufgesunden normotonen Versuchspersonen wurden der Brachial- und Lungenarteriendruck synchron im Wachzustand und im Phenothiazin-Schlaf unter dem Einflus von Adrenalin, Effortil, Vasculat, Noradrenalin, Hypophysin, Acetylcholin, Histamin, Orthostase, Bulbusdruck und Carotissinusentlastung untersucht. Im Gegensatz zu erheblichen Druckveranderungen in der Brachialarterie konnten bei samtlichen Untersuchungen keine vergleichbaren Veranderungen des Pulmonalarteriendruckes gefunden werden.

Functional anatomy of motor recovery after early brain damage

Functional magnetic resonance imaging and transcranial magnetic stimulation were used to examine a 34 year-old right-handed patient, who, at the age of 6 years, had experienced sudden right hemiplegia, seizures, and stupor during a bout of measles encephalitis, followed by incomplete distal right motor recovery. Morphological MRI showed massive unilateral enlargement of the left ventricle, associated with extreme thinning of the white and gray matter, with partial preservation of the pyramidal tract.
