Simultaneous chronic rupture of quadriceps tendon and contra-lateral patellar tendon in a patient affected by tertiary hyperparatiroidism

Abstract Spontaneous ruptures of the extensor mechanism of the knee are very rare. They tend to increase considerably in patients with metabolic diseases such as chronic renal failure, hyperparathyroidism, diabetes, gout, and systemic lupus erythematosus. The reported case regards a 48-year-old man with chronic, spontaneous and simultaneous quadriceps, and contra-lateral patellar tendon rupture. The patient suffered from chronic renal failure and for the past year from tertiary hyperparathyroidism. Ruptured tendons were repaired and both knee were evaluated monthly for the next 12 months. Total Elbow Replacement as Primary Treatment for Complex Fractures of the Distal Osteopenic Humerus

Background: The use of hip and shoulder arthroplasty in elderly patients for grossly displaced intraarticular fractures is becoming more and more accepted. Total elbow replacement (TER) has not been considered an option for the treatment of extensively comminuted fractures of the distal humerus because surgeons feel the outcome is generally not predictable after TER. Evaluation of TER: On the basis of a MEDLINE literature search of this issue and the documented results based on the improvement of implant designs, operative technique and patient selection, TER has a definite role in the treatment of some fractures of the distal humerus.

Orbital Decompression for Endocrine Ophthalmopathy: The Endonasal Approach

A surgical endonasal procedure is described to perform orbital decompression in patients suffering from ophthalmopathy in Grave's disease. The decompression technique employs removal of the lamina papyracea as an exclusive modality or in conjunction with a transmaxillary or lateral decompression approach. This present series contain 23 patients who underwent decompression over a 3-year period and were followed for at least 6 months postoperative. The results of decompression were assessed by measuring reduction in proptosis and visual acuity. All patients demonstrated improved visual acuity. The medial and lateral collateral ligament complexes represent primary elbow ligament stabilizers. Injury to these structures may occur because of an elbow dislocation or due to a severe varus or valgus force. Most elbow dislocations are stable once reduced and may be treated conservatively. Post-synovectomy changes in the articular cartilage

Synovectomy of the left knee was performed in 37 immature rabbits, using the right knee for control. In the first set of experiments the articular cartilage was examined at weekly intervals for 8 weeks following the operation, paying particular attention to the metachromatic changes in the ground substance of the cartilage. In the second set of experiments, restoration of synovium was examined. In the third and fourth set of experiments, the uptake of S35 by the cartilage was assessed using autoradiography and densitometry. In the fifth set of experiments, alterations in S35 uptake by the chondrocitic cells of the cartilage were studied by electron microscopy.

Waterjet dissection in pediatric cranioplasty

Object Waterjet dissection has been shown to separate tissues of different resistance, with preservation of blood vessels. In cranioplasty, separation of subcutaneous tissue and dura mater is often difficult to achieve because the various tissue layers strongly adhere to each other after decompressive craniotomy. Anterior dislocation in a total knee arthroplasty: A case report and literature review.

Dislocations of a total knee arthroplasty (TKA) are an uncommon injury and only few cases of anterior dislocations have been reported. We report a rare case of anterior dislocation of a 10-year-old posterior stabilized total knee arthroplasty in a 74-year-old woman. The patient was successfully treated by close reduction of the dislocation followed by immobilization in full extension with a long leg cast. Because of a high risk of neurovascular complications, a high index of suspicion for vascular injury must be maintained. For this reason, a proper diagnosis is required and immediate reduction is recommended.
