Endoscopic treatment of gastroesophageal reflux disease.

Gastroesophageal reflux disease is a common chronic disorder which has a severe effect on the patient's quality of life. In view of the high cost of medical therapy and the limitations of surgery, a variety of endoscopic techniques have been developed for the treatment of this condition, and these have shown apparently encouraging results, at least in the short term. However, promising results have been obtained in only around two-thirds of patients over a short-term follow-up period of about 6 months. Eight-point compass rose underlay technique in 72 consecutive elderly patients with large incisional hernia

Summary Background Repair of incisional hernia (IH) in the elderly is a challenge for the surgeon. Primary closure is preferable but is not always possible because of high recurrence rates of IH repaired without a prosthesis and/or possible respiratory and cardiovascular complications due to extreme tension of the margins. We report our experience with underlay mesh placement in elderly patients with large IH. Argon laser suture lysis following glaucoma filtering surgery – A short introduction to the procedure

Abstract Background Trabeculectomy is the most commonly performed glaucoma filtering surgery. The success of the outcome of an operation to reduce the intraocular pressure (IOP) is dependent on postoperative wound modulation by antimetabolite therapy and by flow regulation of the filtering bleb, which can be achieved by argon laser suture lysis. Methods and case report Postoperative IOP controls, examination of bleb formation and bleb massage can lead to the decision to perform up to four argon laser suture lyses in order to reduce IOP and enhance aqueous humor flow and bleb formation. Two weeks to 120 months after enucleation of renal tumours the postoperative defects were correctly localized by renal computed tomography in 34 patients. Typical defect morphology was either wedge-shaped or concave. No dependence on tumour localization, tumour size, operative technique or a certain surgeon was observed. The cortical defects were smaller (mean 2.0 cm) than the original, prominent tumours (mean 3.4 cm). In small defects without tension suture of the capsule is sufficient, leading to minor defects. Suture needles in Oral Surgery: Alterations depending on the type and number of sutures

This study examined whether the number and type of sutures used in oral surgery influence two ad hoc variables (incision plane and displaced area), which are two variables related to whether the suture needle is suitable for the task. Seventy-five TB-15 needles were studied, which were used to suture between zero and three mucosa and/subperiosteal sutures, producing 15 groups with 5 needles in each one. A Novel Technique of Posterolateral Suturing in Thoracoscopic Diaphragmatic Hernia Repair

Background Closure of the posterolateral defect in some cases of congenital diaphragmatic hernia (CDH) can be difficult. Percutaneous transcostal suturing is often helpful to create a complete, watertight closure of the diaphragm. A challenge with the technique is passing the needle out the same tract that it entered so that no skin is caught when the knots are laid down into the subcutaneous tissue.
