Salvage treatment with ganciclovir in a splenectomized, polytransfused patient affected by systemic inflammatory response syndrome

A 23-year-old man was admitted to hospital with a 12-day history of daily fever. A clinical history revealed that 10 months previously, the patient had been splenectomized and polytransfused for a severe blunt trauma. On admission, laboratory data revealed significant leukocytosis (33,230/ul). The patient's general clinical conditions rapidly worsened into a severe systemic inflammatory response syndrome in four days. After 10 days of broad-spectrum antibiotic treatment, the temperature curve was unmodified and severe leukocytosis persisted (44,300 ul) with absolute lymphocytosis. Interleukin-6 (IL-6) is a four-helical protein which, on target cells, binds to a specific IL-6-receptor and two molecules of the promiscuous signal transducing protein gp130. Structure-function analysis defined three molecular contact sites between IL-6 and its receptor subunits. Using this information, competitive antagonistic proteins as well as hyperagonistic proteins were developed. Possible therapeutic applications of IL-6 antagonists are in IL-6 dependent haematological disorders (Castleman's disease, POEMS syndrome, multiple myeloma) and bone diseases (Paget's disease, osteoporosis). Este estudio examina la visión de 201 futuros maestros sobre los modelos científicos y sus funciones en ciencias y en su enseñanza. Se diseñó e implementó un cuestionario con cinco preguntas abiertas, de las cuales se analizan dos ¿Qué se entiende por modelo científico? y ¿Para qué consideras que son utilizados los modelos en ciencias? ¿Y en la enseñanza de las ciencias? We address several aspects of lattice QCD calculations of the hadronic vacuum polarization and the associated Adler function. We implement a representation derived previously which allows one to access these phenomenologically important functions for a continuous set of virtualities, irrespective of the flavor structure of the current. Secondly we present a theoretical analysis of the finite-size effects on our particular representation of the Adler function, based on the operator product expansion at large momenta and on the spectral representation of the Euclidean correlator at small momenta. We show how the Majorana representation can be used to express the pure states of an N-qubit system as points on the Bloch sphere. We compare this geometrical representation of N-qubit states with an alternative one, proposed recently by the present authors.
