Dear Sir, We read with interest the comments of Dr. Tfelt-Hansen [1] on the two recently published randomized controlled trials comparing patients’ preference (primary end-point) and efficacy (secondary end-points) of frovatriptan with respect to rizatriptan [2] and almotriptan [3]. In both studies, frovatriptan showed similar preference and short-term efficacy outcomes (pain relief and pain free episodes at 2 h) with respect to the other two triptans. Aim: To evaluate the effect of complete elimination of the calcific deposit on the clinical outcome in patients with a recalcitrant calcifying tendinitis of the shoulder. Patients and methods: 50 patients with a chronic calcifying tendinitis of the shoulder (Type Gartner I or II) underwent a single application of extracorporeal shock waves (3000 impulses, energy flux density 0.60mj/ mm 2 ). Lithotripsy of gallbladder stones in 1992: Improved indications and actual results

ESWL of gallbladder stones and subsequent adjuvant oral dissolution is a safe and comfortable therapy of symptomatic cholecystolithiasis. This method is limited on highly selected 10 to 15% of patients. Recent experience indicates that the ideal indication is given in patients with radiolucent solitary gallstones (diameter ≤2 cm) which are harboured in a gallbladder with unimpaired contractility. 1 year after ESWL 80% of those patients present with a stone free gallbladder. Computed tomography of gallstones may probably improve selection of patients and therapeutic success.

Frovatriptan vs almotriptan for treatment of menstrual migraine: a double-blind, randomized, cross-over, multicenter Italian study

Results 67 of the 96 female patients of the intention-to-treat population of the main study had regular menstrual cycles and were thus included in this subgroup analysis. 77 migraine attacks classified as related to menses were treated with frovatriptan and 78 with almotriptan. Rate of pain relief at 2and 4-hrs was 36% and 53% for frovatriptan and 41% and 50% for almotriptan (p=NS between treatments). Rate of pain free at 2and 4-hrs was 19% and 47% with frovatriptan and 29% and 54% for almotriptan (p=NS). Abstract Background Management of ovarian cysts in infants is controversial; it can be conservative or surgical, and the management is determined by the cyst's size and sonographic features. Methods A surgical approach using a 10-mm umbilically placed operative laparoscope was taken in 3 female infants with antenatally diagnosed large, simple ovarian cysts. An automatic procedure for the online recognition of REM sleep appears to be a necessary tool for selective REM sleep deprivation in depressive patients. To develop such a procedure we applied an artificial neural network to preprocessed single-channel EEG activity. EOG and EMG information was purposely not provided as input to the network. A generalized back-propagation algorithm was used for computer simulation.
