Consenso español de salud física del paciente con depresión

2014

Comorbidity between depression and physical illnesses is very common and has a significant impact on the health and management of the patient. With the support of the Sociedades Espanolas de Psiquiatria y Psiquiatria Biologica, and Sociedad Espanola de Medicos de Atencion Primaria (SEMERGEN) a consensus was prepared on physical health in patients with depression and is summarized in the present work. The literature review highlighted the high frequency of cardiovascular and endocrine-metabolic disorders in patients with depression such as diabetes and obesity, thus making the primary and secondary prevention recommendations for patients with cardiovascular or metabolic risk applicable to pa…

Community acquired pneumonia in children: Outpatient treatment and prevention

2015

There have been significant changes in community acquired pneumonia (CAP) in children in the last decade. These changes are related to epidemiology and clinical presentation. Resistance to antibiotics is also a changing issue. These all have to be considered when treating Community acquired pneumonia (CAP). In this document, two of the main Spanish pediatric societies involved in the treatment of CAP in children, propose a consensus concerning therapeutic approach. These societies are the Spanish Society of Paediatric Infectious Diseases and the Spanish Society of Paediatric Chest Diseases. The Advisory Committee on Vaccines of the Spanish Association of Paediatrics (CAV-AEP) has also been …

Standard Therapeutic Approach and New Therapies

2015

The traditional approach to therapy of Crohn’s disease has been the step-up approach usually represented as a pyramid (Fig. 7.1) where, progressing from mild to severe disease, therapeutic choices proceed step by step from less potent drugs at the basement of the pyramid to more potent but also more toxic drugs at the top. The advent of biological therapies and the wider use of immunomodulators, together with the opportunity to achieve ambitious treatment goals and possibly to modify the course of disease, have led to other approaches such as the accelerated step-up or the top-down approach. This means that immunomodulators and biologics can be used earlier and de-escalated when disease is …

Immunoguiding, the Final Frontier in the Immunotherapy of Cancer

2014

T cells play an important role in cancer. This notion is strongly supported by an enormous number of trials on the clinical impact of tumor-infiltrating T cells and studies consequently showing that therapeutic interventions which are based on transfer, activation and expansion, or de-blocking of tumor-specific T cells, which have met with clinical success. To optimally profit from the flow of newly developed immune-based therapeutics aiming to reinforce the systemic and local tumor-specific T-cell response, it will be required to identify biomarkers that provide the rationale to use a particular therapy, that measure the effect of intervention, and that can predict the outcome of a therape…

Multifocal Repetitive Transcranial Magnetic Stimulation—A Novel Paradigm in Migraine Treatment

2019

Transcranial Magnetic Stimulation (TMS) is a non-invasive brain stimulation method used for analyzing structural and functional interactions in brain, assess cortical reactivity, and map functionally relevant brain regions inducing a controlled current pulse in a defined region of the cortex. From a clinical point of view, TMS has shown promising results in the therapeutic approach in a large number of psychiatric and neurological conditions such as anxiety, depression, OCD, headache disorders—migraine being one of the most encountered, etc. In patients with migraine, the pharmacologic therapy can be divided in abortive and preventive treatment of the attack. Usually the treatment is starte…

Cerebral Autoregulation in Non-Brain Injured Patients: A Systematic Review.

2021

Introduction: Cerebral autoregulation (CA) plays a fundamental role in the maintenance of adequate cerebral blood flow (CBF). CA monitoring, through direct and indirect techniques, may guide an appropriate therapeutic approach aimed at improving CBF and reducing neurological complications; so far, the role of CA has been investigated mainly in brain-injured patients. The aim of this study is to investigate the role of CA in non-brain injured patients. Methods: A systematic consultation of literature was carried out. Search terms included: “CA and sepsis,” “CA and surgery,” and “CA and non-brain injury.”Results: Our research individualized 294 studies and after screening, 22 studies were anal…

Obstructive Sleep Apnea and Snoring

2017

Moderate-to-severe sleep-disordered breathing (SDB) is common in the general population, particularly between ages 50 and 70. Obstructive sleep apnea (OSA) is a major public health concern presenting a high associated risk for hypertension, cardiovascular disease, type II diabetes, and motor vehicle accidents. The dentist should always evaluate for the presence of risk factors in patients with any signs of SDB. PSG is the gold standard for the diagnosis of OSA and sleep disorders. The CPAP device is still considered the most effective therapeutic approach for the management of SDB in both adults and children; however, their side effects and low adherence make clinicians look for alternative…

Therapeutic Advances in Emergency Cardiology: Acute Pulmonary Embolism.

2019

Background Acute pulmonary embolism (PE) requires rapid diagnosis and early and appropriate treatment, often under conditions of hemodynamic instability. The therapeutic strategy should optimally integrate the therapeutic arsenal in a multidisciplinary but unitary approach. Areas of uncertainty The short list of the major uncertainties associated with acute PE should include limited general public awareness on venous thromboembolism, acute hemodynamic support not based on evidence from randomized clinical trials, with few updates lately, mainly linked to extracorporeal membrane oxygenation, thrombolytic therapy having firm indications only in high-risk PE, without clear strategies for parti…

Maximal Strength Training as a Therapeutic Approach in Long-Standing Anorexia Nervosa: A Case Study of a Woman With Osteopenia, Menstrual Dysfunction…

2018

In persons with anorexia nervosa (AN), compulsive exercise and osteopenia are common symptoms. Although treatment of osteopenia besides weight regain is lacking, maximal strength training (MST) has been found to be effective in other populations. Such training has not been prescribed to those with AN due to uncertainty of tolerance. We therefore examined use of MST in a woman with long-standing AN, osteopenia, menstrual dysfunction, and compulsive exercise. The MST intervention consisted of four exercises: three sets of five repetitions maximum (RM), 3 times per week for 16 weeks. We examined muscle strength, bone mineral density (BMD), AN psychopathology, and compulsive exercise at baseli…

The hidden burden of severe asthma: from patient perspective to new opportunities for clinicians

2020

Severe asthma is an important topic in respiratory diseases, due to its high impact on morbidity and mortality as well as on health-care resources. The many challenges that still exist in the management of the most difficult-to-treat forms of the disease, and the acknowledgement of the existence of unexplored areas in the pathophysiological mechanisms and the therapeutic targets represent an opportunity to gather experts in the field with the immediate goals to summarize current understanding about the natural history of severe asthma and to identify gaps in knowledge and research opportunities, with the aim to contribute to improved medical care and health outcomes. This article is a conse…
