Inner ear function in children with Fabry disease

Aim: The prevalence of hearing loss in patients with Fabry disease is still uncertain. This paper examines hearing loss in a group of young patients with Fabry disease. Methods: A clinical ear nose and throat examination,pure-tone air and bone conduction audiometry, speech audiometry and middle ear testing (tympanometry and acoustic reflex testing) were carried out in four girls and two boys with Fabry disease (age, 7-17 years), receiving enzyme replacement therapy (ERT). The role of fear-avoidance cognitions and behaviors in patients with chronic tinnitus.

The current study investigated the role of fear-avoidance-a concept from chronic pain research-in chronic tinnitus. A self-report measure the "Tinnitus Fear-Avoidance Cognitions and Behaviors Scale (T-FAS)" was developed and validated. Furthermore, the role of fear-avoidance behavior as mediator of the relationship between anxiety sensitivity and tinnitus handicap was investigated. Long-Term Results after Interval Therapy with Intratympanic Gentamicin for Meni??re???s Disease

Objectives The new single-shot and interval treatment for Meniere's disease with gentamicin was designed to avoid cochlear damage during treatment with gentamicin. Methods To date, 90 patients were treated with the single-shot or interval gentamicin therapy. Fifty-seven cases of Meniere's disease were followed up prospectively between 2 and 4 years. During one treatment series, a maximum of three intratympanic gentamicin injections within 15 days were applied, each consisting of 0.3 mL (12 mg) of gentamicin (days 1, 8, and 15). If the process invades the cochlea, a sensorineural hearing loss usually takes place. The cochlear implant is a good alternative in these patients. L’audiometria multifrequenziale nello studio degli acufeni

Multifrequency audiometry, using 20 Hz steps between 125 Hz to 800 Hz, studies patients affected by essential tinnitus. Bilateral normal auditive perception, tested using standard audiometers, was not confirmed in all patients analysed using the multifrequency technique. We observed neurosensorial deficit areas between 1040-1100 Hz and between 1920-1960 Hz. These results were integrated with diagnostic imaging and tested coclear nucleus microiscaemic areas and endococlear suffering. The patients who were given medical treatment showed benefits which were confirmed by partial regression of the neurosensorial deficit and tinnitus.

1. Somatosensory tinnitus (objective or subjective) is tinnitus that can be modulated by stimulation of the somatosensory system. 2. Abnormal interactions between the auditory and the somatosensory nervous system that may occur at several levels of the central nervous system cause somatosensory tinnitus. 3. This chapter discusses how administration of a ­botulinum toxin can alleviate tinnitus and the mechanism of its action, and how that relates to its effects on chronic pain. 4. A proven benefit of botulinum toxin in patients with objective tinnitus is also discussed.
