Scarf and Akin osteotomies for moderate and severe hallux valgus

Abstract Background The scarf and the combined scarf-Akin procedures are reliable therapeutic tools and can obtain effective correction of symptomatic moderate to severe hallux valgus deformities. Methods The data from 30 patients (37 feet) with moderate to severe hallux valgus deformity who had scarf osteotomies have been retrospectively reviewed. 32 Akin and 77 Weil osteotomies were also carried out at the same stage. The medial and lateral collateral ligament complexes represent primary elbow ligament stabilizers. Injury to these structures may occur because of an elbow dislocation or due to a severe varus or valgus force. Most elbow dislocations are stable once reduced and may be treated conservatively. However, if the elbow remains unstable surgical treatment is recommended. Repair should be anatomical and should restore stability to the elbow in order to permit an early active range of motion. Radial head fractures are the most common elbow fractures, accounting for 20–30 %. Typically, the fractures evolve from a fall on the outstretched hand with the elbow extended and the forearm in pronation. The role of the radial head as an important stabilizer of the elbow joint has been recognized in the last decade. Sixty percent of the axial load transmitted through the elbow is conducted through the radial head. Furthermore, the radial head is an important stabilizer against valgus stresses. The incorrect adjustment of footwear produces alterations in the foot that affect quality of life. The usual measurements for shoe design are lengths, widths and girths, but these measures are insufficient. The foot presents an angle between the forefoot and the rearfoot in the transverse plane, which is associated with foot pronation, hallux valgus and metatarsus adductus.
