Canine fossa puncture in endoscopic sinus surgery: report of two cases

Abstract Introduction Chronic rhinosinusitis with nasal polyposis is a common chronic disease that often affects maxillary sinus. Endoscopic sinus surgery is the most common procedure for treating the majority of maxillary sinus lesions. Objective To demonstrate the role of canine fossa puncture during endoscopic sinus surgery procedure in patients with severe maxillary sinus disease. Methods We present 2 cases where canine fossa puncture has been performed as method to obtain a complete access to the maxillary antrum. This experimental study used dynamic computed tomography (CT) to assess the effects of chest compressions only during cardiopulmonary resuscitation (CCO-CPR) on alveolar recruitment and haemodynamic parameters in porcine model of ventricular fibrillation. Measurements of the volume and density of intracerebral tumors by CT following therapy.

For the interpretation of curative measures in patients with cerebral tumors CT is of increasing importance. The therapeutic effects can be demonstrated by close follow-up studies without any of the disadvantage of invasive neuroradiological methods. Our investigations of 125 patients with cerebral tumors are based on volume and density determinations. The CT studies of removed or inoperable tumors followed by radiation and/or cytostatic therapy prove that the best results follow a combination of both. Technique and value of gas and pantopaque cisternography in the diagnosis of cerebello-pontine angle tumours

The techniques of gas and Pantopaque cisternography are described, as well as the various advantages and disadventages of these methods. Patients without neurological signs in whom a small tumour is suspected in the region of the cerebellopontine angle should be examined with Pantopaque. The examination can be carried out as an outpatient procedure. If definite neurological signs are present, gas cisternography is preferable in order to demonstrate secondary displacements of the ventricular system as well as the tumour itself.

Endo-neuro-sonography: anatomic aspects of the ventricles.

To evaluate the usefulness of transendoscopic sonography, we have studied the use of a new sonographic probe of 6 F diameter in 11 fresh specimens. We achieved a precise imaging of well known anatomic structures and, moreover, obtained an additional dimension in endoscopy, since the sonographic probe adds a transverse scan to the endoscopic view, like a mini-CT at the tip of the probe. In this way, we also examined the guiding characteristics of this imaging technique, both in real time and on-line. Our results promise further interesting aspects of this technique in minimally invasive neurosurgery and suggest that further development and clinical experience seem to be justified.

Disconnected pancreatic duct syndrome: complete pancreas transection secondary to acute pancreatitis

Disconnected pancreatic duct syndrome is a serious complication of acute pancreatitis which is defined by a complete discontinuity of the pancreatic duct, such that a viable side of the pancreas remains isolated from the gastrointestinal tract. This pancreatic disruption is infrequently observed in the clinical practice and its diagnostic and therapeutic management are controversial. We present an extreme case of disconnected pancreatic duct syndrome with complete duct disruption and pancreatic transection following acute pancreatitis, as well as the diagnostic and therapeutic processes carried out.
