Patient-Specific Rehearsal Feasibility Before Endovascular Repair of Ruptured Abdominal Aortic Aneurysm.

2019

Purpose: To evaluate the feasibility of a patient-specific rehearsal (PsR) before emergency endovascular aneurysm repairs (eEVAR) and its influence on the operation. Materials and Methods: From February 2016 to October 2016, 10 consecutive patients (mean age 75 +/- 7.4 years; 9 men) presenting with a ruptured abdominal aortic aneurysm (rAAA) suitable for standard EVAR were enrolled in the study. A 3-dimensional (3D) model of the abdominal aorta was generated on a virtual reality simulator based on the patient's computed tomography (CT) images. Following the patient-specific simulation setup, PsR was conducted during patient admission or in parallel with the preoperative eEVAR workup. Measur…

Particle image velocimetry study of the celiac trunk hemodynamic induced by continuous-flow left ventricular assist device

2017

Abstract Whereas left ventricular assist device (LVAD) is the gold-standard therapy for patients with heart failure, gastrointestinal bleeding is one of the most common complications. LVAD implantation may remarkably impact aortic hemodynamics so that experimental and computational flow analyses can be used to study the disease mechanisms. Here we present an experimentally-calibrated computational model of the celiac trunk hemodynamic of a LVAD-supported patient who experienced bleeding after device implantation. Specifically, both particle image velocimetry (PIV) and echocardiography were used to measure and compare flow distributions in each branch of a phantom model of the patient abdomi…

Shear stress alterations in the celiac trunk of patients with a continuous-flow left ventricular assist device as shown by in-silico and in-vitro flo…

2017

Background The use of left ventricular assist devices (LVADs) to treat advanced cardiac heart failure is constantly increasing, although this device leads to high risk for gastrointestinal bleeding. Methods Using in-silico flow analysis, we quantified hemodynamic alterations due to continuous-flow LVAD (HeartWare, Inc., Framingham, MA) in the celiac trunk and major branches of the abdominal aorta, and then explored the relationship between wall shear stress (WSS) and celiac trunk orientation. To assess outflow from the aortic branch, a 3-dimensional-printed patient-specific model of the celiac trunk reconstructed from an LVAD-supported patient was used to estimate echocardiographic outflow …

Obstructive uropathy following aortofemoral graft surgery.

1991

Fifty patients who had undergone aorto-bifemoral bypass with a bifurcated Dacron graft for aortoiliac arteriosclerotic obliteration were examined with real-time sectorial ultrasound to screen for the presence of hydronephrosis. Hydronephrosis was detected in only 1 case. The low incidence of this complication in our cases may be due to the fact that all our patients were explored at least 1 year after surgery. Some may have developed hydronephrosis early, but this may have resolved spontaneously during the first year after the operation, thus remaining undiagnosed. Pathogenic mechanisms suggested for the development of hydronephrosis and therapeutic approaches are discussed. We conclude tha…

Detection of a Dissecting Aortic Aneurysm by Renal Transplant Imaging

1997

After 2 years with a normal post-transplant course a 45-year-old man showed an elevated creatinine level (4.5 mg/dL). Transplantation had been neccessary because of progressive glomerulonephritis. Renal transplant imaging was obtained with 150 Mbq Tc-99m MAG3. The perfusion images showed radionuclide accumulation in the area of the upper abdominal aorta and delayed perfusion of the right iliac artery. Accumulation of Tc-99m MAG3 in the transplant was slow and no filling of the bladder could be observed. Immediately after imaging, the patient developed acute hemorrhagic shock. At emergency surgery, a long dissecting and ruptured aneurysm of the abdominal aorta was seen causing delayed and re…

Cardiovascular dysfunction and vitamin D status in childhood acute lymphoblastic leukemia survivors

2018

Vitamin D (25-OHD) has a role in bone health after treatment for cancer. 25-OHD deficiency has been associated with risk factors for cardiovascular disease, but no data focusing on this topic in childhood cancer survivors have been published. We investigated the 25-OHD status in children treated for acute lymphoblastic leukemia (ALL), and evaluated its influence on vascular function. 25-OHD levels were evaluated in 52 ALL survivors and 40 matched healthy controls. Patients were grouped according to 25-OHD level (< 20 ng/m or ≥ 20 ng/ml). Auxological parameters, biochemical and hemostatic markers of endothelial function (AD, HMW-AD, ET-1, vWFAg, TAT, D-dimers, Fbg, and hs-CRP), ultrasound ma…

Endograft repair of spontaneous infrarenal abdominal aortic dissection

2005

A 71-year-old man with a history of hypertension and a recent myocardial infarction experienced a sudden periumbilical and back pain. Abdominal tenderness was found at physical examination, with no changes in electrocardiogram and serial enzyme determinations. Computed tomographic angiography (CTA) showed a normal thoracic aorta, an infrarenal aortic dissection extending into the left common iliac artery and a contralateral iliac occlusive disease (A, Cover). Although antihypertensive therapy was administered, the patient had persistent pain; subsequently, he underwent endovascular repair with a bifurcated stent graft (Talent, Medtronic Ave, Santa Rosa, Calif) introduced via the left femora…

Investigation of the hemodynamic flow conditions and blood-induced stresses inside an abdominal aortic aneurysm by means of a SPH numerical model.

2019

The estimation of blood flow-induced loads occurring on the artery wall is affected by uncertainties hidden in the complex interaction of the pulsatile flow, the mechanical parameters of the artery, and the external support conditions. To circumvent these difficulties, a specific tool is developed by combining the aorta displacements measured by an electrocardiogram-gated-computed tomography angiography, with the blood velocity field computed by a smoothed particle hydrodynamics (SPH) numerical model. In the present work, the SPH model has been specifically adapted to the solution of the 3D Navier-Stokes equations inside a domain with boundaries of prescribed motion. Images of the abdominal…

The compliance modelling of the pathology aorta of the type of aneurism

2015

The Abdominal Aorta Aneurysm (AAA) is a pathology that is defined by a localized and permanent dilation of the artery and which involves over 8.8% of the seniors. Currently, when a patient has a dilatation of the aorta leading to a surgery because of the rupture risk, the therapeutic decision is made depending on the diameter of the aneurysm. To determine this diameter, it is usually conducted an examination by medical imaging (ultrasound, CT, MRI...). However, it notes that the diagnosis can’t be satisfied with a single dimensional measurement face to induced risks: first of all, when the diameter exceed a certain growth, the risk of rupture can reach 50% but more than 5% of surgical proce…

Minimum cause--maximum effect: the travelogue of a bullet.

2010

This case report involves a 57-year-old male, accidentally shot in the chest with a small bore firearm. The bullet entered the left hemithorax, disrupting the left internal mammarian artery. It then penetrated the anterior wall of the right ventricle causing a pericardial tamponade. After leaving the base of the right heart it perforated the diaphragm, the liver, the spleen and the pancreas. Finally, it penetrated the abdominal aorta 3 cm proximally to the coeliac trunk and reached its final position paravertebrally. This case report illustrates that the management of even minimum gunshot wounds requires a maximum variety of surgical skills. Keywords: Thoracoabdominal injury; Shotgun wound;…
