Abstract Aim Aim of the current study was to prospectively evaluate the potential role of peripheral circulating CD34+ stem cells as new independent marker of appropriate hemopoietic balance in patients with thalassemia major and intermedia. Materials and methods Peripheral blood samples from patients with thalassemia major (TM) and intermedia (TI) were drawn. Surgery of the esophagus may be indicated for benign and malignant pathologies. Esophageal cancer represents the most frequent indication for surgical treatment, and the important nutritional concern arisen in relationship between cancer, surgery, and nutritional status of operated oncological patients has been explored by a vast scientific literature. Among benign pathologies indicating surgery involving the esophagus, achalasia has been largely studied, with surgery representing one of several effective therapeutic options.

Achalasia Secondary to Submucosal Invasion by Poorly Differentiated Adenocarcinoma of the Cardia, Siewert II: Consideration on Preoperative Workup.

Secondary achalasia due to submucosal invasion of cardia by gastric cancer is a rare condition. We report a case of pseudoachalasia, secondary to the involvement of gastroesophageal junction by poorly differentiated gastric cancer, initially mistaken as idiopathic form. We focus on the difficulty to establish differential diagnosis only on the basis of routine exams and we stress the necessity of “second level” instrumental exams; EUS in routine workup in selected patients should be considered. HISTORY AND CLINICAL FINDINGS A 78-year-old woman suffered from achalasia since 63 years with a progressive decompensation over the last year. 53 years ago, treatment with the Stark Dilator and 24 years ago, pneumatic dilation had been carried out. Currently, the patient presented with dysphagia for liquid and solid food, with permanent retrosternal pain and regurgitation for every meal, leading to a weight loss of 10 kg. INVESTIGATIONS The barium esophagogram showed a marked dilation of the esophagus with retinated secretions and food. The cardia had a maximum width of 15 mm. The article analyses letters exchanged in the years 1930 to 1949 between two outstanding Czech poets - Jan Zahradníček and František Halas. Despite their contrary views (Zahradníček was perceived as an ardent Catholic, while Hałas – as holding rather left-wing worldviews), their written communication is a testimony of their friendship which lasted for two decades.
