Investigational agents for Crohn's disease.

2010

IMPORTANCE OF THE FIELD: Increased understanding of the biological mechanisms of Crohn's disease has opened the door to a large number of new molecules; some of these are approved for clinical use, while others remain under evaluation. In this review, we examine the clinical efficacy of all the new drugs that have been evaluated in controlled trials in the last 12 years. AREAS COVERED IN THIS REVIEW: Anti-TNF therapy has been reviewed briefly, given the many comprehensive reviews on this topic; attention is focused mainly on the other biological therapies. In assessing the clinical efficacy of these molecules, we consider only the remission rate, as this is considered the most meaningful en…

Human Papillomavirus (HPV) Detection in Keratotic Variants of Oral Lichen Planus: Negative Results in a Preliminary Open Study

2017

Background During the last few decades, several researches have been identified HPV genomic sequences in Head Neck Squamous Cell Carcinomas (HNSCC) and in Oral Potentially Malignant Disorders (OPMD) hypothesizing a possible viral etiological role. However, the reported prevalence of HPV-DNA (varying from 0% to 100%) in these lesions result still very discordant. Objective Human papillomavirus (HPV) infection in Oral Lichen Planus (OLP) has been largely investigated but not so extensively with respect to its morphological/histopathological features. The possible influence of hyper-keratosis/parakeratosis in HPV detection for keratotic variants of OLP is here proposed and investigated. Materi…

Papillon Lefevre syndrome: bridge between Dermatologist and Dentist

2010

Papillon-Lefevre syndrome is a rare autosomal recessively inherited condition which shows features common to both dentistry and dermatology. This disease is characterized by palmoplantar hyperkeratosis and severe periodon- titis. Therefore both the dentist and the dermatologist should be aware of the same because an early diagnosis of the syndrome can help to preserve the teeth by early institution of treatment, using a multidisciplinary approach. Here we present a case report of Papillon-Lefevre syndrome, along with a comprehensive review of the etiology, pathology, clinical features, differential diagnosis and management of the condition.

Resumption of Ventilation at the End of Obstructive Sleep Apneas is not Determined by Diaphragmatic Fatigue

1988

In patients affected by obstructive sleep apnea syndrome (OSAS) a progressive increase of the force developed by the respiratory muscles is usually observed during the occlusive phase before upper airway patency is resumed (Onal and Lopata, 1986).

Communicating with the hepatobiliary surgeon through structured report

2019

Communicating radiological findings to hepatobiliary surgeons is not an easy task due to the complexity of liver imaging, coexistence of multiple hepatic lesions and different surgical treatment options. Recently, the adoption and implementation of structured report in everyday clinical practice has been supported to achieve higher quality, more reproducibility in communication and closer adherence to current guidelines. In this review article, we will illustrate the main benefits, strengths and limitations of structured reporting, with particular attention on the advantages and challenges of structured template in the preoperative evaluation of cirrhotic and non-cirrhotic patients with fo…

Hepatocellular carcinoma: Diagnostic imaging criteria

2020

The diagnosis of hepatocellular carcinoma (HCC) can be reached noninvasively on imaging in patients at high risk as discussed in chapter “Hepatocellular carcinoma: Diagnostic Guidelines.” In this chapter, we describe the CT and MR imaging criteria more commonly used in the guidelines for the diagnosis and management of patients with HCC, including arterial phase hyperenhancement, washout, capsule, growth, and hypointensity on hepatobiliary phase MR imaging. The provided definitions are based on the latest version (v2018) of the liver imaging reporting and data system (LI-RADS) document (American College of Radiology 2018, https://www.acr.org/Clinical-Resources/Reporting-and-Data-Systems/LI-…

Precapillary Servo Control of Blood Pressure and Postcapillary Adjustment of Flow to Tissue Metabolic Status

1996

Background There are several shortcomings in current understanding of how the microvasculature maintains tissue homeostasis. Presently unresolved issues include (1) integration of the potentially conflicting needs for capillary perfusion and hydrostatic pressure regulation, (2) an understanding of signal transmission pathways for conveying information about tissue energetic status from undersupplied tissue sites to the arterioles, (3) accounting for the experimentally observed interrelations between precapillary and postcapillary resistances, and (4) an explanation of how precise local adjustment of perfusion to metabolic demands is achieved. Methods and Results A novel conceptualization o…

Fraktur des Os capitatum bei einem 11-jährigen Jungen

2010

We report the case of an isolated non-displaced fracture of the capitate in an 11-year-old boy. On the basis of the relevant literature we present a survey of the diagnostic and treatment options. To the best of our knowledge this is the first description of a paediatric capitate fracture in the German-speaking area.
