Isthmic Spondylolisthesis is Associated with Less Revisions for Adjacent Segment Disease After Lumbar Spine Fusion Than Degenerative Spinal Condition…

2021

Objective: We aim to compare the rate of revisions for adjacent segment disease (ASD) after lumbar spine fusion (LSF) surgery between patients with isthmic spondylolisthesis (IS) and degenerative lumbar spine disorders (DLSD). Summary of Background Data: ASD is a major reason for late reoperations after LSF surgery. Several risk factors are linked to the progression of ASD, but the understanding of the underlying mechanisms is imperfect. If IS infrequently becomes complicated with ASD, it would emphasize the role of the ongoing degenerative process in spine in the development of ASD. Methods: 365 consecutive patients that underwent elective LSF surgery were followed up for an average of 9.7…

Interrelationships between oxidative stress and MMP-9 in subjects with severe obstructive sleep apnea syndrome

2016

Obstructive sleep apnea syndrome (OSAS) is significantly and independently associated with an increased risk of cardiovascular diseases, cerebrovascular events and all-cause mortality and some studies have demonstrated that the incidence of cardiovascular events is related to its severity. Our aim was to examine the oxidative status and the matrix metalloproteases (MMP) profile in a group of subjects with OSAS. We enrolled 48 subjects with OSAS, defined after a 1-night cardiorespiratory sleep study, which were subsequently subdivided in two subgroups according to the severity of OSAS: mild-moderate OSAS = “Low”, severe OSAS = “High”. We measured the parameters of oxidative stress, such as l…

Achalasie oder Pseudoachalasie? Fallstricke der Diagnostik und Therapieentscheidung

2007

History Patient 1 (female, aged 55 years) had for some time complained of morning nausea. She reported symptoms of reflux with regurgitation of food for two-and-a-half years and also dysphagia with retrosternal bolus obstruction for the last eighteen months. Patient 2 (male, aged 84 years) complained of restrosternal dysphagia with each intake of food for one year, weight loss of 12 kg and occasional regurgitation of food. Investigations The general condition of patient 1 was only slightly impaired but that of patient 2 markedly reduced. Routine laboratory tests were unremarkable in both. Barium meal in patient 1 revealed fixed narrowing in the region of the esophageal hiatus. The inferior …

Preliminary study of the neuromuscular activation during barefoot and shod running

2015

Chromoendoscopy and Magnifying Endoscopy in Patients with Gastroesophageal Reflux Disease

2004

Gastroesophageal reflux disease (GERD) is common in the Western world. Upper endoscopy is needed to characterize the disease. Barrett’s esophagus as a complication of GERD is an established precancerous condition which can lead to adenocarcinoma in the distal esophagus. This review summarizes recent advances in the endoscopic characterization of Barrett’s esophagus using magnification endoscopy and chromoendoscopy. Methylene blue, indigo carmine and acetic acid are commonly used dyes to facilitate diagnosis of Barrett’s esophagus. Methylene blue is absorbed in the specialized columnar epithelium, which is pathognomonic for Barrett’s esophagus. Indigo carmine and acetic acid are used as cont…

Hepatitis C virus: Current steps toward elimination in Germany and barriers to reaching the 2030 goal

2021

Abstract Hepatitis C virus (HCV) affects over 70 million people globally, with an estimated 399 000 HCV‐related deaths in 2016. The World Health Organization (WHO) has set a goal to eliminate HCV by 2030. Despite the availability of direct‐acting antivirals—highly effective and well‐tolerated therapies for HCV—many patients infected with HCV in Germany have not initiated treatment, including a majority of those who are aware of their positive diagnosis. Barriers to screening, diagnosis, and treatment are major factors taking many countries off track for HCV elimination by 2030. Identifying country‐specific barriers and challenges, particularly in at‐risk populations such as people who injec…

Spectral decomposition of RR-variability obtained by an open loop parametric model for the diagnosis of neuromediate syncope

2002

The role of the cardiovascular regulatory mechanism in patients with neuromediate syncope (NS) is poorly understood. The aim of this study was to accomplish continuous non-invasive analysis of the baroreflex mechanism in patients during a head-tip tilt-table test (HTT) using an open-loop autoregressive model with exogenous input. The model describes the causal dependence of the RR interval on the systolic arterial pressure (SAP) variability. Thus, RR variability results as the linear composition of SAP-dependent (Pdep) and SAP-independent parts of the RR power (P). Further, the model allows the estimation of the baroreflex gain using the modulus of the transfer function (G) from SAP to RR i…

Synchronization index for quantifying nonlinear causal coupling between RR interval and systolic arterial pressure after myocardial infarction

2002

The analysis of nonlinear couplings among cardiovascular variability series can improve the knowledge of the cardioregulatory mechanism and help to understand how it can be affected by pathologies. In this study, the influences of acute myocardial infarction (AMI) on the causal relationships between the heart period and the arterial pressure were investigated by a nonlinear dynamic approach based on the corrected cross-conditional entropy. Whereas the global synchronization index did not differentiate the post-AMI patients from the young and old control groups, the causal indexes evidenced the impairment of the baroreflex control and showed an increase of the mechanical driving of the RR in…
