European Society of Hypertension guidelines for blood pressure monitoring at home: a summary report of the second international consensus conference …

2008

This document summarizes the available evidence and provides recommendations on the use of home blood pressure monitoring in clinical practice and in research. It updates the previous recommendations on the same topic issued in year 2000. The main topics addressed include the methodology of home blood pressure monitoring, its diagnostic and therapeutic thresholds, its clinical applications in hypertension, with specific reference to special populations, and its applications in research. The final section deals with the problems related to the implementation of these recommendations in clinical practice. ispartof: Journal of Hypertension vol:26 issue:8 pages:1505-1530 ispartof: location:Neth…

Vestibular compensation of otolith graviceptive dysfunction in stroke patients

2022

BACKGROUND AND PURPOSE A sensitive and frequent clinical sign of a vestibular tone imbalance is the tilt of the perceived subjective visual vertical (SVV). There are no data yet focusing on lesion location at the cortical level as a factor for predicting compensation from the tilt of the SVV. METHODS With modern voxelwise lesion behavior mapping analysis, the present study determines whether lesion location in 23 right-hemispheric cortical stroke patients with an otolith dysfunction could predict the compensation of a vestibular tone imbalance in the chronic stage. RESULTS Our statistical anatomical lesion analysis revealed that lesions of the posterior insular cortex are involved in vestib…

Stress and recovery assessment during simulated microgravity: Effects of exercise during a long-term head down tilt bed rest in women.

2009

International audience; The aim of this study was to determine the effects of a 60-day head-down tilt long-term bed rest (HDT) on stress and recovery in sixteen healthy female volunteers. Participants were randomly assigned to either an exercise group (Exe) that followed a training program combining resistive and aerobic exercises, or to a no-exercise control group (Ctl). Psychological states were assessed using the Rest-Q, a validated questionnaire based on stress-recovery responses. A longitudinal analysis revealed significant changes in the general and specific-stress scales for all participants throughout the experiment with a critical stage from supine to standing posture leading to a …

Sympathetic Overactivity and 24-Hour Blood Pressure Pattern in Hypertensives with Chronic Renal Failure

1995

In order to assess the activity of the sympathetic system and to evaluate the 24-h blood pressure pattern in hypertensives with chronic renal failure (CRF), 12 CRF patients and 16 essential hypertensives (EHs) were studied. In all subjects, plasma samples for catecholamines and renin activity were obtained both in the basal condition and after standing, and 24-h blood pressure monitoring (ABPM) was performed. The 24-h mean blood pressure results were quite similar between CRFs and EHs. In 50% of the CRFs, ABPM showed a nighttime decrease in diastolic BP (DBP) greater than 10%, while in the remaining 50% the ABPM indicated a nondipper blood pressure pattern. Of the 16 EHs, 4 had a nighttime …

Vascular Health Assessment of The Hypertensive Patients (VASOTENS) Registry: Study Protocol of an International, Web-Based Telemonitoring Registry fo…

2016

Background: Hypertension guidelines recommend ambulatory blood pressure (ABP), central aortic pressure (CAP), and pulse wave velocity (PWV) as parameters for estimating blood pressure (BP) control and vascular impairment. Recent advances in technology have enabled devices to combine non-invasive estimation of these parameters over the 24-hour ABP monitoring. However, currently there is limited evidence on the usefulness of such an approach for routine hypertension management. Objective: We recently launched an investigator-initiated, international, multicenter, observational, prospective study, the Vascular health Assessment Of The Hypertensive patients (VASOTENS) Registry, aimed at (1) eva…

Blood Pressure Monitoring in Cardiovascular Disease

2017

While the practice of taking blood pressure readings at the physician’s office continues to be valid, home blood pressure monitoring is being increasingly used to enhance diagnostic accuracy and ensure a more personalized follow-up of patients. In the case of white coat hypertension and resistant arterial hypertension, ambulatory blood pressure monitoring is indispensable. Recent studies attach great importance to nocturnal blood pressure patterns, with a reduction in these becoming a treatment goal, a strategy known as chronotherapy. Home blood pressure monitoring is useful for both diagnosis and follow-up of arterial hypertension. Its use, particularly if combined with other patient-suppo…

Removal of primary tumor improves survival in metastatic breast cancer. Does timing of surgery influence outcomes?

2010

Resection of intact primary tumor is controversial in metastatic breast cancer patients. The aim of this study is to review the impact of surgical resection of primary tumor on overall survival and to assess the role of timing of surgery on survival rates.208 patients with metastatic breast cancer diagnosed between 1982 and 2005 in the Hospital Clinico of Valencia (Spain) were analysed. Exclusion criteria were age80, PS 3-4, Charlson score 3 or follow-up90 days. 123 of these underwent surgery and 85 did not. In order to assess the role of timing, the "surgery" cohort was divided into two sub-groups: "before" (n = 78) or "after" (n = 45) diagnosis of disseminated disease. In the surgery group…

European guidelines on perioperative venous thromboembolism prophylaxis:Day surgery and fast-track surgery

2017

: In recent years, day surgery and fast-track surgery have experienced a continuous increase in volume. Many procedures are now performed on an outpatient protocol, including general, orthopaedic, oncological, reconstructive or vascular surgery. The management of these patients is safe, but the incidence of venous thromboembolism in this population remains unknown. Several risk factors can be identified and stratified derived from studies of inpatient surgical management (e.g. Caprini score). Recommendations for thromboprophylaxis should be tailored from the assessment of both personal and procedure-related risk factors, although with a lack of evidence for application in outpatient managem…
