Principles and Technique of Foam Sclerotherapy and Its Specific Use in the Treatment of Venous Leg Ulcers

Invention of foam sclerotherapy has significantly changed the current phlebological practice. Compared with liquid sclerosants, obliterating foam is more efficient, especially for the closure of larger veins. This review discusses clinical aspects of foam sclerotherapy with a focus on its use for the treatment of venous leg ulceration, including the rationale for its use in the treatment of these chronic wounds, physicochemical mechanisms responsible for stability and disintegration of sclerosant foam, pathomechanism of neurologic adverse events seen after foam sclerotherapy, and techniques that can increase efficacy of this procedure and lower frequency of adverse events.

In the postoperative period patients are at risk of excessive oxygen consumption (VO2). However, patients suffering from cardiovascular disease may be unable to increase their oxygen transport capacity sufficiently and may be especially vulnerable to tissue hypoxia as part of the reaction to intraoperative stress. Using kinetic modelling for shaping profile dialysis, we present a promising approach to improve the cardiovascular stability of patients during dialysis treatment. In order to obtain an insight into the physiological mechanisms of increased stability, a model considering alterations of electrolytes and water distribution and of acid-base status was developed. This algorithm was used for the evaluation of 114 dialysis sessions, which were performed with highly individualised profiles. Three salts of 5-amino-2-sulfonamide-1,3,4-thiadiazole (Hats) were prepared and characterized by physico-chemical methods. The p-toluensulfonate, the methylsulfonate, and the chlorhydrate monohydrate salts of Hats were evaluated as carbonic anhydrase (CA, EC 4.2.1.1) inhibitors (CAIs) and as anticonvulsants and diuretics, since many CAIs are clinically used as pharmacological agents. The three Hats salts exhibited diuretic and anticonvulsant activities with little neurotoxicity. The human (h) isoforms hCA I, II, IV, VII, IX, and XII were inhibited in their micromolar range by these salts, whereas pathogenic beta and gamma CAs showed similar, weak inhibitory profiles.

Abstract There have been limited studies to date targeting gaseous elemental mercury (GEM) flux from soil emission in enriched volcanic substrates and its relation with CO 2 release and tectonic structures. In order to evaluate and understand the processes of soil–air exchanges involved at Solfatara of Pozzuoli volcano, the most active zone of Campi Flegrei caldera (Italy), an intensive field measurement survey has been achieved in September 2013 by using high-time resolution techniques. This study provides the first micro-thermometric data of fluid inclusions (FIs) in mafic loose (disaggregated) xenocrysts and ultramafic xenoliths in explosive products of the melilitite-carbonatite Mt. Vulture volcano (southern Italy). Pure CO2 late stage FIs hosted in rock-forming minerals of wehrlite xenoliths and clinopyroxene xenocrysts were trapped at the local crust-mantle boundary (32 km).
