Phacoemulsification and Vitrectomy with the Erbium: or Neodymium:YAG-Laser

2002

Summary There are an increasing number of applications for Erbium:YAG and Neodymium:YAG lasers in opthalmology. Laser phacoemulsification and Erbium-laser vitrectomy are of major interest. We report the underlying technology and more than 1-year-experience using the PHACOLASE (Carl Zeiss Meditec, Jena, Germany, 10–100 Hz frequency, 5–50 mJ single pulse energy) for phacoemulsification and pars plana vitrectomy and the Lyla-Dodick laser photolysis (A.R.C. Laser, Eckental-Forth, Germany, 1–20 Hz, up to 8 mJ single pulse energy). The Erbium:YAG laser and Neodymium:YAG laser enabled us to extract lenses with up to medium hard nuclei. Moreover, the Erbium laser enabled fast vitrectomy with little…

The posterior chamber phakic refractive lens (PRL): a review

2012

Implantation of phakic intraocular lenses (pIOLs) is a reversible refractive procedure, preserving the patient’s accommodative function with minimal induction of higher order aberrations compared with corneal photoablative procedures. Despite this, as an intraocular procedure, it has potential risks such as cataracts, chronic uveitis, pupil ovalization, corneal endothelial cell loss, pigmentary dispersion syndrome, pupillary block glaucoma, astigmatism, or endophthalmitis. Currently, only two models of posterior chamber pIOLs are commercially available, the implantable collammer lens (STAAR Surgical Co.) and the phakic refractive lens (PRL; Zeiss Meditec). The number of published reports on…

Evaluation of the improvement of sensibility with the new IOL Master software version 5.4.3

2009

Purpose To evaluate the improvement of sensibility of the new IOL-Master version 5.4.3 compared with the version 4.08. Methods 214 eyes of 115 patients were examined in a cataract preoperative evaluation making use of both IOL-Master software versions 5.4.3 and 4.08. The type and severity of the lens opacification was assessed by slitlamp biomicroscopy. Results In 182 of the eyes examined both versions were capable to determine the AL; in 29 the version 4.01 could not determine AL whereas the version 5.4.3 was able to measure it; these cases showed posterior capsular cataract (16), dense nuclear cataract (11), posterior synechiae (1) and asteroid hyalosis (1). Finally, the AL of 3 eyes coul…

Disturbi emozionali nel trattamento chirurgico della cataratta

2010

Aim.The aim of this study was to appraise the degree of phychic uneasiness and anxiety in subjects who undergo surgical intervention of the cataract and the degree of satisfaction for the result of the interventetion.Methods.A sample of 50 subjects(22 males and 28 females)has been evaluated before the intervention on the cataract: the level of anxiety was measured by means of the Hamilton Anxiety Rating Scales T(HARS) and the degree of psychic uneasiness and psychopathological (phobia, somatization, depression) demonstrations by means of the test of psychic uneasiness-questionnaire IGD.The evalution of the degree of satisfaction for the results of the intervention has been obtained by means o…

Trifocal toric intraocular lenses in eyes with low amount of corneal astigmatism.

2020

Aim To evaluate the refractive and visual outcomes following cataract surgery and implantation of a trifocal toric intraocular lens (IOL) in eyes with low degrees of corneal astigmatism. Methods Twenty-six eyes of 22 patients who underwent implantation a trifocal toric IOL (FineVision PODFT, PhysIOL s.a., Liege, Belgium) were enrolled. Phacoemulsification with femtosecond laser, capsular tension ring insertion and intraoperative aberrometry were performed in all cases. All IOLs used showed a cylinder power of 1.00 D. Main outcome measures were refractive error and corrected-distance visual acuity (CDVA) and uncorrected-distance visual acuity (UDVA) values. Eyes were evaluated at 4mo post-su…

Presbyopia and the aging eye: Existing refractive approaches and their potential impact on dry eye signs and symptoms.

2020

Every part of the human body is subject to aging, including the eye. Increased prevalence of dry eye disease with age is widely acknowledged: aging threatens ocular surface homeostasis, altering the normal functioning of the lacrimal functional unit and potentially leading to signs and symptoms of dry eye. Parallelly, two additional degenerative processes take place within the crystalline lens, leading to presbyopia and cataractogenesis. With continuously increasing life expectancies both conditions are expected to impact society further. Correction strategies for presbyopia and cataracts either directly or indirectly challenge ocular surface additionally. On one hand, contact lens presbyop…

Intravitreal dexamethasone implant versus anti-vascular endothelial growth factor therapy combined with cataract surgery in patients with diabetic ma…

2021

Objective: to compare outcomes of cataract surgery combined with either anti-Vascular Endothelial Growth Factor (anti-VEGF) therapy or dexamethasone implant (DEX) in patients with diabetic macular oedema (DMO). Methods: Pubmed and Embase databases were searched for studies reporting outcomes of diabetic cataract surgery combined with either anti-VEGF or DEX, with a follow-up ≥3 months. The primary outcome was the mean change in central macular thickness (CMT). Mean change in best corrected visual acuity (BCVA) was considered as a secondary outcome. The mean difference between baseline and post-treatment values (MD) with 95%-Confidence Interval (95%CI) was calculated and meta-analyses were p…

Kosteneffektivitätsanalyse der Versorgung von Kataraktpatienten mit monofokalen Intraokularlinsen aus Sicht der Leistungserstatter

2003

BACKGROUND Data on the cost effectiveness of cataract surgery with respect to the German statutory health insurance system are hardly available. Hence the increase in visual acuity as primary efficacy endpoint, order of posterior capsule opacification as primary complication endpoint and direct cost for supplementation with monofocal intraocular lenses have been assessed via meta-analyses to evaluate the procedure's cost effectiveness from the perspective of the German statutory health insurance system. MATERIAL AND METHODS Using the medical internet database MEDLINE two meta-analyses on publications between 1995 and 2002 were carried out, one estimating the gained increase in visual acuity…

1999

Abstract Objective To objectively measure and compare halo, flicker, and glare disability in pseudophakic eyes with monofocal (MONO) and multifocal (MULTI) intraocular lenses (IOLs) with respect to the influence of corneal surface quality, astigmatism, and age. Design Prospective case series. Participants This clinical trial involved 28 eyes of 28 patients after small-incision cataract surgery with a MONO silicone IOL and 28 eyes of 28 patients with zonal-progressive silicone IOL. Intervention A computer program objectively determined halo, glare, and flicker. Corneal surface quality and astigmatism were measured using computerized videokeratography. Ray-tracing analysis was performed based…

Parasite-induced change in host behaviour and susceptibility to predation in an eye fluke–fish interaction

2004

Abstract Trophically transmitted parasites may increase their transmission efficiency by altering the behaviour of infected hosts to increase their susceptibility to predation by target hosts (the next host in the life cycle). The parasite Diplostomum spathaceum (Trematoda) reduces the vision of its fish intermediate hosts: its metacercariae lodge themselves in the eyes of fish and induce cataract formation, which gives them the opportunity to affect fish behaviour. We examined whether D. spathaceum eye flukes change the preference of fish for the surface layers of the water column or their escape behaviour, which could make the fish more vulnerable to predation by bird hosts. We also studi…
