Video-assisted surgery: suggestions for failure prevention in laparoscopic cholecystectomy

Background: Surgery differs from other medical specialties in its execution. It is often complex and includes considerable individual variations. Observing problems in operating theatres (OT) allows for the identification of system failures which should be defined for learning purposes to increase patient safety and enhance general safety culture within hospital organizations. Akute emphysematöse Cholezystitis als Ursache eines Pneumoperitoneums

A 48-year-old man developed progressively more severe epigastric pain, pain on pressure in the right upper abdomen and fever up to 38.6 degrees C so that acute cholecystitis was suspected. Ultrasound did not demonstrate a gall-bladder but a sickle shaped, dense echo with a distal adjoining echo-free zone. Computed tomography revealed air in the gall-bladder lumen as well as intramural and pericholecystic air pockets, findings pathognomonic for emphysematous cholecystitis. In addition pneumoperitoneum was diagnosed. Subsequent cholecystectomy intraoperatively revealed a gangrenous, nonperforating gall-bladder in which E. coli was demonstrated. Visā pasaulē, tajā skaitā arī Latvijā, viena no biežākajām ķirurģiskajām saslimšanām, kam nepieciešama operatīva ārstēšana, ir žultsakmeņu slimība (ŽAS) jeb holelitāze. Tās izplatība pieaug līdz ar iedzīvotāju vecumu un mūsdienu dzīvesveida paradumiem, tādiem kā, nesabalansēts, neregulārs uzturs un mazkustīgs dzīvesveids. Šis bakalaura darbs tika izveidots ar mērķi - noskaidrot iedzīvotāju zināšanas par žultsakmeņu slimības attīstību veicinošajiem faktoriem un profilaksi.
