Early diagnosis of primary melanoma of caecum. Case report and review of the literature.

Malignant gastrointestinal melanoma is usually a metastatic lesion. We report the case of a 79-year-old female asymptomatic patient. Colonoscopy revealed one plane 15 mm pigmented lesion in the caecum. Neither CT scan of the abdomen nor right hemicolectomy revealed no metastatic disease. Histopathological examination of the surgical specimen was indicative of malignant melanoma.A set of additional enquires such as laboratory and imaging tests did not point out any suspicious lesions in the skin, eye, leptomeninges or other sites. Materials and methods Data collected from 20 patients undergoing right hemicolectomy for right bowel cancer were evaluated. Patients included were divided in two groups of 10 subjects respectively, according their age. In the first group were allocated patients of age ≥ 70 years, whereas in the second group were included subjects of age ≤ 60 years. Patients affected by postoperative delirium or presenting complications that could altered the regain of intestinal motility were excluded. The time of first passage of flatus, the time of first defecation and the postoperative stay were evaluated.

Findings in 23 children with neuronal intestinal dysplasia (NID) are presented. Twelve children had Hirschsprung's disease that masked the existence of proximal NID. In all cases the diagnosis was established by histological-histochemical examination. Amelioration of symptoms by conservative treatment alone was achieved in 4 children; in 13 cases colostomy was necessary. In 6 patients colon resection was performed: 2 suffered from both colitis and obstruction; the other 4 were treated for obstruction, multiple perforations, persistant adynamia of the left colon, and fistulation.
