Early endovascular aneurysm repair after percutaneous coronary interventions

2015

Objective The objective of this study was to report long-term results of early endovascular aortic aneurysm repair after percutaneous coronary intervention (PCI). Methods This was a retrospective analysis of all patients presenting with abdominal aortic aneurysm and coronary artery disease treated during the same hospitalization by endovascular aortic aneurysm repair performed soon after PCI. Primary outcomes were perioperative mortality, perioperative complications, survival after treatment, and freedom from reintervention. Results A total of 20 patients were included, and all completed both procedures. No deaths or abdominal aortic aneurysm ruptures occurred between the PCI and the aortic…

Acute Hemodynamic Effects of Nicorandil in Coronary Artery Disease

1992

Nicorandil is a potent vasodilator with antianginal and anti-ischemic properties that acts on both the coronary and the peripheral vascular bed. Because of its dual vasodilatory mechanisms mediated by an increase in cyclic GMP similar to that of nitrates and by a selective increase in the K+ conductance of the smooth muscle cell membrane, nicorandil unloads the right and left ventricles at rest and during exercise. Thus, compared with the classic nitrates, nicorandil is a more balanced vasodilator, i.e., it affects not only the venous capacitance vessels (as predominantly affected by nitrates) but also the arterial resistance vessels. In clinical pharmacologic trials, nicorandil has been ad…

Comparison of genetic risk prediction models to improve prediction of coronary heart disease in two large cohorts of the MONICA/KORA study

2021

Abstract It is still unclear how genetic information, provided as single‐nucleotide polymorphisms (SNPs), can be most effectively integrated into risk prediction models for coronary heart disease (CHD) to add significant predictive value beyond clinical risk models. For the present study, a population‐based case‐cohort was used as a trainingset (451 incident cases, 1488 noncases) and an independent cohort as testset (160 incident cases, 2749 noncases). The following strategies to quantify genetic information were compared: A weighted genetic risk score including Metabochip SNPs associated with CHD in the literature (GRSMetabo); selection of the most predictive SNPs among these literature‐co…

Retinal vascular imaging in cardiovascular medicine: New tools for an old examination

2017

The old concept that the eye is the only window through which the body microcirculation can be observed in life easily, safely and repeatedly is part of the cultural baggage of every physician. When Hermann von Helmholtz invented the direct ophthalmoscope in 1850, he opened the door to a non-invasive visualization of human microcirculation in vivo at the retina. Only a few years later, the Scottish ophthalmologist Marcus Gunn documented the close association between retinal changes, and renal and cardiovascular diseases (CVD) in hypertensive patient. In 1939, Keith, Wagener, and Barker showed that the severity of retinal microvascular abnormalities was predictive of mortality in patients wi…

Physical exercise and Sport activities in patients with and without coronary heart disease

2016

Background: The quantity and intensity of physical activity required for the primary prevention of coronary heart disease remain unclear. Therefore, we examined the association between physical activity and coronary risk. Methods: We studied 100 patients with chest pain, 78 men and 22 women, not older than 65 years, admitted to a coronary care unit. Patients were subdivided in 3 groups: the first group included patients with acute myocardial infarction, the second group included patients with chronic heart disease, the third included patients with non-ischemic chest-pain. A questionnaire on daily physical activity was filled by each patient. Results: A significantly higher percentage of pat…

Chronic burden of near-roadway traffic pollution in 10 European cities (APHEKOM network).

2013

AD; International audience; Recent epidemiological research suggests that near road traffic-related pollution may cause chronic disease, as well as exacerbate related pathologies, implying that the entire “chronic disease progression” should be attributed to air pollution, no matter what the proximate cause was. We estimated the burden of childhood asthma attributable to air pollution in 10 European cities by calculating the number of cases of 1) asthma caused by near road traffic-related pollution, and 2) acute asthma events related to urban air pollution levels. We then expanded our approach to include coronary heart diseases in adults.Derivation of attributable cases required combining c…

A New Technical Approach For Retrograde Administration of Cardioplegic Solutions

1989

Myocardial protection via the coronary sinus is now currently used by several groups. Although it has generally provided satisfactory results, some of its problems are still not completely resolved. We present a new technique of cardioplegia delivery through the coronary sinus with a Pezzer catheter inserted into it and secured in place by a purse string suture. We believe that this method is safer and more reliable than others.

Prognostic Value of Troponins in Patients With or Without Coronary Heart Disease: Is it Dependent on Structure and Biology?

2020

Convincing evidence has emerged that cardiac troponins (cTns) T and I are the biochemical gold standard for diagnosing cardiac injury, and may also be used as efficient screening and risk stratification tools, especially when measured with the new high-sensitivity (hs-) immunoassays. In this narrative review, we aim to explore and critically discuss the results of recent epidemiological studies that have attempted to characterise the prognostic value of cTns in patients with or without cardiovascular disease, and then interpret this information according to cTn biology. Overall, all recent studies agree that higher blood levels of cTns reflect the larger risk of cardiovascular events and/or…
