ABSTRACT Introduction The dipeptidyl peptidase-4 inhibitors (DPP-4is), which belong to the class of incretin-based medications, are recommended as second or third-line therapies in guidelines for the management of type 2 diabetes mellitus. They have a favorable drug tolerability and safety profile compared to other glucose-lowering agents. Effect of vildagliptin compared to glimepiride on postprandial proinsulin processing in the β cell of patients with type 2 diabetes mellitus

This study compared the effect of Glimepiride versus Vildagliptin on β-cell function and the release of intact proinsulin (PI) in patients with type 2 diabetes mellitus. Patients on metformin monotherapy were randomized to add on treatment with Vildagliptin or Glimepiride. A standardized test meal was given at baseline, after 12 and 24 weeks of treatment. Insulin, PI and blood glucose values were measured in the fasting state and postprandial for 300 min. Fasting PI levels significantly decreased in the Vildagliptin group. The small island of Lampedusa, a key destination of the Central Mediterranean Route connecting Africa to Italy, offers a special observatory on the contemporary trans-Mediterranean odyssey of migrants, although often transformed into a “border spectacle.” Upon landing, migrants are stripped of their belongings, as these are impounded by the authorities. Such an act of dispossession is intended to deprive them of their histories, family ties and cultural identity. Photographer Mario Badagliacca has portrayed a selection of these lost and retrieved items in his work Fragments (2013). Introduction: Type 2 diabetes mellitus (T2DM) is associated not only with high direct healthcare costs, but also with indirect costs, as diabetic complications and the disease itself result in loss of productivity. Vildagliptin is a novel dipeptidyl peptidase-4 inhibitor that is given either alone or in combination with oral hypoglycemic drugs, including metformin. I nuovi uffici municipali di Castelvetrano si contraddistinguono per la sapiente articolazione compositiva sia dell’impianto planimetrico sia della stereometria della fabbrica. I grandi scavi operati sul volume, le sregolature delle bucature sconnesse del fronte laterale, il ritmo irregolare dei pieni e dei vuoti, sono elementi di una interessante complessità, che trova una sintesi indubbiamente significativa nell’essenzialità del volume che le accoglie.
