Perioperative Chemotherapy in Poorly Differentiated Neuroendocrine Neoplasia of the Bladder: A Multicenter Analysis

2020

There is scant evidence about optimal management of poorly differentiated neuroendocrine carcinoma of the bladder (BNEC). We performed a multicenter retrospective study on BNEC patients from 13 Italian neuroendocrine-dedicated centers to analyze strategies associated with better outcomes. Mixed adeno-neuroendocrine carcinomas (MANEC) were included. We analyzed overall survival (OS) in the overall cohort, relapse-free survival (RFS) in radically operated patients and progression-free survival (PFS) in patients who received chemotherapy for metastatic disease. Fifty-one BNEC patients were included (male: 46, median age: 70 years). Overall, median OS was 16.0 months, radical tumor resection wa…

The Perceived Role of Technology in Career Guidance among Practitioners Who are Experienced Internet Users

2011

The increasing use of technology is placing new demands on career guidance practitioners. This article examines what changes, if any, have occurred in the perceptions of guidance practitioners regarding their role and the role of the internet in meeting guidance goals and delivering career guidance services. The data were collected in focus groups in 2001–2002 and a follow-up study in 2010. A total of seven focus groups were held. The data were analysed using combined methods. The results indicated that practitioners now observe that the need for differentiated service delivery modes is more explicit due to varied levels of readiness in decision-making and ICT literacy.

Five Years of Game Programming Outreach : Understanding Student Differences

2014

This paper presents lessons learned from five years of teaching a five-day game design and programming outreach course. The course was offered in summer time and targeted at middle and high school students. In total, 462 youngsters have taken part in 21 course instances. We describe our course concept, and discuss our successes and challenges. In particular, we focus on understanding our student populations by presenting descriptives and statistics of the events, and performing a statistical cluster analysis based on pre- and post-surveys. The cluster analysis was complemented with an analysis of the qualitative data, also originating from the surveys. Taken together, students could be clas…

Pancreatic undifferentiated carcinoma with osteoclast-like giant cells is genetically similar to, but clinically distinct from, conventional ductal a…

2017

Undifferentiated carcinoma of the pancreas with osteoclast-like giant cells (UCOGC) is currently considered a morphologically and clinically distinct variant of pancreatic ductal adenocarcinoma (PDAC). In this study, we report clinical and pathological features of a series of 22 UCOGCs, including the whole exome sequencing of eight UCOGCs. We observed that 60% of the UCOGCs contained a well-defined epithelial component and that patients with pure UCOGC had a significantly better prognosis than did those with an UCOGC with an associated epithelial neoplasm. The genetic alterations in UCOGC are strikingly similar to those known to drive conventional PDAC, including activating mutations in the…

Selinexor in Advanced, Metastatic Dedifferentiated Liposarcoma: A Multinational, Randomized, Double-Blind, Placebo-Controlled Trial

2022

PURPOSE Antitumor activity in preclinical models and a phase I study of patients with dedifferentiated liposarcoma (DD-LPS) was observed with selinexor. We evaluated the clinical benefit of selinexor in patients with previously treated DD-LPS whose sarcoma progressed on approved agents. METHODS SEAL was a phase II-III, multicenter, randomized, double-blind, placebo-controlled study. Patients age 12 years or older with advanced DD-LPS who had received two-five lines of therapy were randomly assigned (2:1) to selinexor (60 mg) or placebo twice weekly in 6-week cycles (crossover permitted). The primary end point was progression-free survival (PFS). Patients who received at least one dose of st…

External drivers of EU differentiated cooperation: How change in the nuclear nonproliferation regime affects member states alignment

2023

Since its establishment, the Common Foreign and Security Policy (CFSP) has strived to increase convergence among EU member states. Yet, convergence remains elusive and scholars have started to explain the emergence of differentiated cooperation resulting from multiple internal EU crises. We posit that the convergence in the EU member states with respect to nuclear weapons has been fundamentally altered by the humanitarian turn to nuclear disarmament. This has led to a crystallization of differentiated subgroups among the member states, whose membership coincides with that of informal groupings active in the broader nuclear nonproliferation regime. Combining quantitative data on resolution s…

Il regionalismo differenziato di cui all'art. 116, co. 3, della Costituzione: un tentativo di inquadramento sistematico nel Titolo V della Costituzio…

2021

The paper analyzes the origins and characteristics of the differentiated re- gionalism referred to in art. 116.3 of the Constitution, highlighting the impact that its implemen- tation could have in Title V of the Constitution, in relation to both ordinary regions and regions with special status. The widespread nature of differentiated regionalism could, in particular, lead to the definitive, tough gradual, overcoming of regional uniformity, towards a model of mainly differentiable regionalism.

Packet management techniques for measurement based end-to-end admission control in IP networks

2000

End-to-end Measurement Based connection Admission Control (EMBAC) mechanisms have been proposed to support real-time flows quality of service requirements over a Differentiated Services Internet architecture. The EMBAC basic idea is to decentralize the admission control decision, by requiring each individual user to probe the network path during flow setup, and by basing the accept/reject decision on the probing traffic statistics measured at the destination. In conformance with the differentiated services framework, routers are oblivious to individual flows and only need to serve data packets with a higher priority than probing traffic. In this paper, we build upon the observation that som…

An API for Advanced Traffic Control in Diffserv Routers

2003

Distributed per-flow admission control is a promising solution for Differentiated Services networks. Its deployment in DiffSery domains requires the ability to suitably configure, in each network router, low-level packet forwarding mechanisms, such as packet dropping algorithms driven by traffic measurements. In this paper we first show that performance effectiveness is achieved by means of non traditional configuration of the forwarding mechanisms. Hence, we propose a modular Application Program Interface that allows to flexibly and adaptively configure the forwarding/dropping behavior associated to a router’s output queue, well beyond the traditional RED/RIO active queue management scheme…

An analytical model of a new packet marking algorithm for TCP flows: Preliminary insights

2004

In differentiated services networks, packet marking schemes can be devised to differentiate packets belonging to a same TCP flow, with the goal of improving the experienced performance. This paper presents an analytical model for an adaptive packet marking scheme proposed in our previous work. The model combines three specific sub-models aimed at describing i) the TCP sources aggregate ii) the marker, and iii) the network status. Some preliminary simulative results seem to validate the model.
