Physiological incongruity of the humero-ulnar joint: a functional principle of optimized stress distribution acting upon articulating surfaces?

Investigations into the distribution of subchondral bone density in the human elbow have suggested that the geometry of the trochlear notch deviates from a perfect fit with the trochlea, and that the load is transmitted ventrally and dorsally rather than through the centre of the humero-ulnar joint. We therefore decided to make a quantitative assessment of the degree of incongruity between the two components in 15 human specimens (age distribution 60 to 93 years) with different types of joint surface. Lateral Ligament Reconstruction

The lateral collateral ligament complex represents the primary lateral elbow ligament stabilizers. In cases of lateral elbow instability, surgical treatment is recommended. Repair should be anatomical and should restore stability to the elbow in order to permit an early active range of motion. Anatomic repair of soft tissue avulsions from bone can be performed with trans-osseous suture or suture anchors. Reconstruction may be required in severe cases and where primary repair is not possible. This chapter describes the surgical techniques for repair and reconstruction of the lateral collateral ligament of the elbow.

Acute (0-2h) and delayed (2-8D) effects of exercise-induced muscle damage and soreness on elbow target movements.

The aim was to examine the acute and delayed effects of exercise-induced muscle damage and soreness on elbow target movements (TM) performance and control. Ten males performed an exercise of 50 maximal eccentric elbow actions. TMs were performed at three movement ranges. Maximal forces, active stretch reflex and TM were tested, and muscle soreness, creatine kinase and elbow joint stiffness were determined acute (after and 2 h) and delayed (2, 4, 6, 8d) postexercise. Both the long lasting muscle soreness and force drop were observed after the exercise. Joint stiffness was increased at 2 h postexercise. Radial head fractures are the most common elbow fractures, accounting for 20–30 %. Typically, the fractures evolve from a fall on the outstretched hand with the elbow extended and the forearm in pronation. The role of the radial head as an important stabilizer of the elbow joint has been recognized in the last decade. Sixty percent of the axial load transmitted through the elbow is conducted through the radial head. Furthermore, the radial head is an important stabilizer against valgus stresses. Kinematics of the ulna during pronation and supination in a cadaver study: implications for elbow arthroplasty

Objective. Aim of this study was to exactly describe and quantify kinematics of the ulna during pro- and supination. Design. Biomechanical study in fresh frozen cadavers. Background. A previous MRI study revealed a varus/valgus motion of the ulna averaging 7.1° during pro-/supination. Axial rotation, however, could not be quantified. Methods. BACKGROUND Previous studies have shown that the trochlear notch is deeper than necessary for an exact fit with the humerus. However, humero-ulnar joint space width and contact areas have so far not been quantified for variations in the load and angle of flexion. METHODS Six fresh cadaveric specimens were investigated at 30 degrees, 60 degrees, 90 degrees, and 120 degrees of flexion and at loads of 25 and 500 N, simulating resisted elbow extension. Ultrasound is a fast, reliable and radiation-free method for the assessment of a wide range of pathological conditions, as well as for the guidance of percutaneous interventional procedures around the elbow, hand and wrist. Intraarticular and periarticular interventional procedures can be easily performed under continuous ultrasound monitoring to ensure correct needle positioning and medication delivery to a specific target.
