Diamond-Shaped Mini-Craniotomy: A New Concept in Neurosurgery

Abstract Background We describe, step by step, a modified, less invasive, diamond-shaped mini-craniotomy that optimizes dural opening and can be performed in elderly patients affected by acute subdural or intracerebral hematomas, in therapy with antiplatelets or anticoagulants. Methods We retrospectively analyzed the clinical records of 67 patients (mean age of 78.5 years) treated in our institution, during a period of 10 years, with this novel diamond-shaped craniotomy. Seventeen patients were treated for intracerebral hemorrhage and 50 patients for acute subdural hematomas. All the patients were in therapy with antiplatelets or anticoagulants. Adaptation and Refinement of Validated Risk Screening Tools in the Spanish Healthcare System

Abstract The aim of this study is to develop a tool to screen elders at risk of hospital readmissions adapted to Spanish target population. The development of this new tool is based on the validated instrument The Community Assessment Risk Screen – CARS –, which is being applied in a sample of 1.000 older patients. The adaptation and refinement of CARS is being carried out through the compilation of new potential variables from primary care clinicians to predict patients at risk of readmissions. The introduction of this new instrument adapted to Spanish target population will provide a potential tool to identify prematurel y patients at risk of hospital readmissions.

The study aims to identify human walking disorders, proposing a model to solve these disorders, regardless of what caused them. Material and method Clinical and somatometric tests, pedometric investigations and sensors were used. An outpatient trial was conducted for a period of 6 weeks with a frequency of application of the recovery program of 3 times / week. Recent data, even in elderly patients, has demonstrated that elevated body mass index affords a worse long-term prognosis, although the magnitude of this relationship weakens as one ages. Community-onset Pseudomonas aeruginosa urinary sepsis in elderly people: Predictive factors, adequacy of empirical therapy and outcomes.

Purpose To know the predictive factors for Pseudomonas aeruginosa (PA) urinary sepsis in hospitalised elderly patients coming from community, the adequacy of empirical antibiotic therapy and its outcomes. Methods Retrospective case-control study conducted between 2007 and 2017. Patients aged 65 years or older with PA urinary sepsis were included as cases. Three age-matched controls, with urinary sepsis caused by other microorganisms, were selected for each case. Predictors for PA urinary sepsis were determined by logistic regression analysis. Asthma is frequent among older people. Nevertheless, under-recognition, misdiagnosis and under-treatment are still relevant issues. We aim to provide an overview of epidemiology of asthma in the elderly, and a thorough description of its pathology and clinical presentation, with special emphasis on the distinction of late versus early-onset asthma. Delirium is a transitory and reversible syndrome which is potentially avoidable; nevertheless represents the most common acute encephalic dysfunction in critically ill patients. Despite this population being daily treated by anesthesiologists in the operating rooms and by critical care physicians in the Intensive care unit, delirium remains an underdiagnosed condition that is associated with a significative increase in morbidity and mortality.
