The endocrine response to severe burn trauma.

The endocrine system is frequently altered after a major burn trauma. Besides the endocrine response to stress characterized by hypercortisolism, several hypothalamus-hypophysis-target gland axes are rapidly perturbed within a few days. These alterations can persist in the long term and deserve an appropriate treatment. Disturbances in water clearance and glucidic metabolism are also common and need to be diagnosed and corrected to decrease morbidity in such patients. Bone and mineral metabolism is deeply compromised and requires correction of mineral abnormalities in order to improve symptoms and prevent bone loss. However, 20% of obese subjects are metabolically healthy, and many studies suggest that fat distribution, in particular abdominal fat excess, is the most important factor that determines a secretion of adipose products that may increase CV risk.

Seasonal Variations on the Cortisol Concentration of the Subretinal Fluid in Rhegmatogenous Retinal Detachment

Probes of the subretinal fluid were obtained from 115 nonselected retinal detachments, operated on during the period from November 19, 1973 to December 20, 1974. All patients had no known endocrine abnormalities and had not ever received topical or systemic corticosteroid therapy. Subretinal fluid Cortisol was determined by competitive protein binding analysis. A statistically significant increase in mean subretinal fluid Cortisol concentration was found for the winter season (3.3 µg/100 ml), as compared with the summer months (1.5 µg/100 ml).
