Topiramate and Metabolic Acidosis in Infants and Toddlers

Summary:  Purpose: Topiramate (TPM) inhibits carbonic anhydrase, with metabolic acidosis as a possible side effect, although this has been reported in only two adult cases. We investigated the acid–base metabolism in infants and toddlers treated with TPM. Methods: Nine infants and toddlers aged 5 months to 2.3 years (median, 6 months) were treated with TPM at maximal doses of 8.2–26 mg/kg/day (median, 11 mg/kg/day). The maximal TPM dose was achieved after 8–35 days (median, 17 days). TPM was given in addition to other antiepileptic drugs (AEDs) in five cases and as a sole AED in four patients with refractory epilepsy resistant to multiple AEDs. The Tortricidae described by J. C. Fabricius (Lepidoptera)

The identity and nomenclature of the 88 species of Tortricidae (Lepidoptera) described by J. C. Fabricius are reviewed. Type material deposited in the Natural History Museum Denmark is illustrated. Lectotypes for Tinea compositella (Fabricius, 1775), Pyralis rivellana (Fabricius, 1775) and P. strigana (Fabricius, 1775) are designated. Two new synonymies are proposed: Pyralis marmorana (Fabricius, 1798), syn. n., of Ancylis achatana (Denis and Schiffermuller, 1775), and P. rusticana (Fabricius, 1794), syn. n., of Epinotia solandriana (Linnaeus, 1758). This article analyzes two novels set in Alexandria. The first one is "Cortile a Cleopatra" by the Italian writer Fausta Cialente. The second one is "Nobody sleeps in Alexandria" (Lâ ahad yanâm fî'l-Iskandariyya) by Ibrahim Abdel Meguid. The analysis focus on actions that characterize the genre of travel literature in which the traveler is expected to perform two actions: leaving then returning. If the traveler does not come back to the starting point, his experience is not considered as "travel", rather as "change of residence". But it does not always work out that way. The Patellofemoral Arthroplasty: The Great Solution, the Great Problem

Unfortunately, many orthopedic surgeons believe that the definite solution for eliminating anterior knee pain in a young patient with multiple operations is the patellofemoral prosthesis (“The Great Solution”). But, they fail to ask themselves the most important question; what is causing the pain? When a patellofemoral prosthesis is implanted in these young patients without finding out the cause of the pain, a more serious problem could be created with a worse solution (“The Great Problem”). The cases we present spotlight this abuse.

Cathepsin D activity levels in colorectal cancer: Correlation with cathepsin B and L and other biological and clinical parameters

Cathepsin D, B and L activity levels were determined in colorectal cancer and correlated with a number of biological and clinical parameters. Our studies have evidenced significant higher activity levels of these lysosomal enzymes in tumor cytosol compared to paired normal mucosa as well as an evident increase of tumor specific cathepsin D activity in Dukes' stage A tumors compared to later stages (B, C and D). Cathepsin D content and activity were determined in matched paired sets of colorectal tumor tissue and normal mucosa and correlated with a number of biological and clinical parameters. Significantly higher cathepsin D activity was measured in tumor cytosol compared to paired normal mucosa (p < 0.02), in Dukes’ stage A tumors compared to Dukes’ B and C (p < 0.05), in tumors < 5 cm compared to those > 5 cm, or in tumors with a low proliferation rate compared to those with a high proliferation rate (p < 0.05).
