Cost-Utility Analysis Applied to the Treatment of Burn Patients in a Specialized Center
2007
Objective To discover the total costs and quality of life of burn patients in a specialist center classified by diagnosis-related groups (DRGs). Design Prospective study of 5-year follow-up from January 1, 1997, through December 31, 2001. Setting Burn Center of Valencia. Patients A total of 898 patients treated at the Burn Center of Valencia. Main Outcome Measures Hospital, extrahospital, caregiving, labor, and social costs of the burn patients grouped by DRG (code 457: extensive burns without operating room procedure; code 458: nonextensive burns with skin graft; code 459: nonextensive burns with wound debridement or other operating room procedure; code 460: nonextensive burns without oper…
Acute stress impairs reward positivity effect in probabilistic learning
2019
Decision making based on feedback learning requires a series of cognitive processes, including estimating the probability of particular outcomes and modulating expectations between expected versus actual outcomes. It has been suggested that stress affects decision making and subsequent processing of feedback valence and magnitude. However, less is known about the effect of acute stress on reward expectancy. In the current study, participants performed a probabilistic learning task, in which they learned an association between response and feedback within different reward expectancy trials (30% and 70%) under the conditions of stress (threat of shock) and safety (no shock). We recorded event…
All-cause and disease-specific mortality among male, former elite athletes: an average 50-year follow-up.
2014
Aim To investigate life expectancy and mortality among former elite athletes and controls. Methods HR analysis of cause-specific deaths sourced from the national death registry for former Finnish male endurance, team and power sports athletes (N=2363) and controls (N=1657). The median follow-up time was 50 years. Results Median life expectancy was higher in the endurance (79.1 years, 95% CI 76.6 to 80.6) and team (78.8, 78.1 to 79.8) sports athletes than in controls (72.9, 71.8 to 74.3). Compared to controls, risk for total mortality adjusted for socioeconomic status and birth cohort was lower in the endurance ((HR 0.70, 95% CI 0.61 to 0.79)) and team (0.80, 0.72 to 0.89) sports athletes, a…
Ready for eHealth? Health Professionals' Acceptance and Adoption of eHealth Interventions in Inpatient Routine Care.
2017
eHealth interventions can be effective in treating health problems. However, adoption in inpatient routine care seems limited. The present study therefore aimed to investigate barriers and facilitators to acceptance of eHealth interventions and of online aftercare in particular in health professionals of inpatient treatment. A total of 152 out of 287 health professionals of various professional groups in four inpatient rehabilitation facilities filled out a self-administered web-based questionnaire (response rate: 53%); 128 individuals were eligible for further data analysis. Acceptance and possible predictors were investigated with a complex research model based on the Unified Theory of Ac…
A decision tree to help determine the best timing and antiretroviral strategy in HIV-infected patients.
2011
SUMMARYOptimal antiretroviral strategies for HIV-infected patients still need to be established. To this end a decision tree including different antiretroviral strategies that could be adopted for HIV-infected patients was built. A 10-year follow-up was simulated by using transitional probabilities estimated from a large cohort using a time-homogeneous Markov model. The desired outcome was for patients to maintain a CD4 cell count of >500 cells/mm3without experiencing AIDS or death. For patients with a baseline HIV viral load ⩾5 log10copies/ml, boosted protease inhibitor-based immediate highly active antiretroviral therapy (HAART) allowed them to spend 12% more time with CD4 ⩾500/mm3than…
IL-10 and TNF-α polymorphisms in a sample of sicilian patients affected by tuberculosis: implication for ageing and life span expectancy
2003
Abstract Human longevity seems to be directly correlated with optimal functioning of the immune system, suggesting that some genetic determinants of longevity might reside in those polymorphisms for the immune system genes that regulate immune responses, in particular cytokine gene polymorphisms. In fact, modification of cytokine network is a constant report in studies on age related modification of immune response. Moreover cytokine polymorphisms studies are indicating their involvement in the reshaping of cytokines network as an integral part of the scenario related to a successful ageing. A particular role might be attributed to the influence of cytokine polymorphisms on the efficiency o…

What are the roles of calorie restriction and diet quality in promoting healthy longevity?
2014
Epidemiological and experimental data indicate that diet plays a central role in the pathogenesis of many age-associated chronic diseases, and in the biology of aging itself. Data from several animal studies suggest that the degree and time of calorie restriction (CR) onset, the timing of food intake as well as diet composition, play major roles in promoting health and longevity, breaking the old dogma that only calorie intake is important in extending healthy lifespan. Data from human studies indicate that long-term CR with adequate intake of nutrients results in several metabolic adaptations that reduce the risk of developing type 2 diabetes, hypertension, cardiovascular disease and cance…
Influence of Teacher and Family Support on University Student Motivation and Engagement
2021
Although many studies endorse the notion that the way students perceive support influences their engagement, very few have explored the possible mediator role of intention to learn between these variables. The present work provides new evidence to the existing literature because it analyses the work of intention to learn (measured with expectancy–value beliefs and achievement goals) as a mediating motivational variable in the relation between university students’ external support (teacher and family) and their engagement. The Educational Situation Quality Model (MOCSE, its acronym in Spanish) has employed as a theoretical framework to perform this analysis. A sample of 267 Spanish universit…
Air pollution in European countries and life expectancy—modelling with the use of neural network
2019
Abstract The present paper discusses a novel methodology based on neural network to determine air pollutants’ correlation with life expectancy in European countries. The models were developed using historical data from the period 1992–2016, for a set of 20 European countries. The subject of the analysis included the input variables of the following air pollutants: sulphur oxides, nitrogen oxides, carbon monoxide, particulate matters, polycyclic aromatic hydrocarbons and non-methane volatile organic compounds. Our main findings indicate that all the variables significantly affect life expectancy. Sensitivity of constructed neural networks to pollutants proved to be particularly important in …
