Knowledge and management of temporomandibular joint disorders by general dentists in Spain

2019

Background Given the importance of temporomandibular disorders (TMD), we tried to assess general dentists’ knowledge regarding etiology, diagnosis, and treatment in order to understand their attitude. Material and Methods A sample of 130 general dentists answered a 16-item questionnaire on three areas – etiology, diagnosis, and management of common temporomandibular disorders – as well as a question on the need for continuous education regarding TMD management in common clinical practice in Spain. Given that the descriptive statistics achieved reflect significantly different values among means in each area, a variance analysis for repeated measurements was applied in order to contrast diffe…

The role of the dentist in the diagnosis and management of patients with oral mucosal diseases

2021

Based on a few case reports of oral mucosal diseases a number of questions is raised about the role of dentists-general practitioners in the diagnostic procedure and management of patients with such diseases. For instance, are dentists prepared to prescribe topical corticosteroids and should dentists be taught how and when to take a biopsy? And how about palpation of the neck? A strong recommendation is made to take clinical pictures for proper documentation and, if needed, for telediagnostic procedures. Another issue relates to the communication between dentists and dental specialists when dealing with patients with oral diseases. In case of a patient suffering from burning mouth syndrome …

Dental approach of orofacial pain in head and neck cancer patients

2018

Background Orofacial pain (OFP) is an undesirable sensation frequently associated with head and neck cancer (HNC) and its treatment. OFP negatively impacts the quality of life of oncological patients. The approach to OFP diagnosis and management can differ if the patient visits a dentist or physician. The aim of this study was to present a case series of HNC with OFP managed by a dentist team and to discuss its role in the management of OFP. Material and methods We recruited twenty-two adult patients with OFP and previous diagnosis of HNC referred to an academic dental clinic from 2015 to 2017. Results Nociceptive was more frequent than mixed and neuropathic pain, however 54,4% of the cases…

Continuous neurophatic orofacial pain: a retrospective study of 23 cases

2016

Background To determine the clinical characteristics of Continuous Neuropathic Orofacial Pain in patients that suffer Persistent Idiopathic Facial Pain (PIFP), Painful Post-Traumatic Trigeminal Neuropathy (PPTTN) or Burning Mouth Syndrome (BMS) and to describe their treatment. Material and Methods A retrospective observational study was made, reviewing the clinical history of the patients diagnosed with Continuous Neuropathic Orofacial Pain between 2004 and 2011 at the Orofacial Pain Unit of the Master of Oral Surgery and Implantology of the University of Barcelona and at the Orofacial Pain Unit of the Teknon Medical Center of Barcelona. Results The average age of the patients with Continuo…

Pregabalin in the treatment of inferior alveolar nerve paraesthesia following overfilling of endodontic sealer

2014

A case of orofacial pain and inferior alveolar nerve (IAN) paraesthesia after extrusion of endodontic sealer within the mandibular canal treated with prednisone and pregabalin is described. A 36-year-old woman underwent root canal treatment of the mandibular second right premolar tooth. Post-operative panoramic radiograph revealed the presence of radiopaque canal sealer in the mandibular canal. Damage to IAN consecutive to extrusion of endodontic sealer was diagnosed. Non-surgical management was decided, including: 1 mg/kg/day prednisone 2 times/day, once-daily regimen, and 150 mg/day pregabalin, two doses per day, monitoring the progress with periodic follow-up visits. Six weeks after the …

Case-control study of craniomandibular disorders in patients with fibromyalgia

2015

Background: Fibromyalgia is a clinical syndrome characterized by chronic widespread pain, which is non-articular and is predominantly experienced in the muscles and vertebral column, and by extensive heightened sensitivity to local pressure at many specific points The purpose of this study was to measure differences in the level of painful symptoms and in the movements of the mandible in a group of patients who had been diagnosed as suffering from fibromyalgia, in comparison with a control group. The anxiety and subjective pain levels and their relation with mandibular mobility were also compared. Materials and Methods: A case-control study was designed. The temporomandibular joints and mas…

Chronic neuropathic facial pain after intense pulsed light hair removal. Clinical features and pharmacological management

2015

Intense Pulsed Light (IPL) photodepilation is usually performed as a hair removal method. The treatment is recommended to be indicated by a physician, depending on each patient and on its characteristics. However, the use of laser devices by medical laypersons is frequent and it can suppose a risk of damage for the patients. Most side effects associated to IPL photodepilation are transient, minimal and disappear without sequelae. However, permanent side effects can occur. Some of the complications are laser related but many of them are caused by an operator error or mismanagement. In this work, we report a clinical case of a patient that developed a chronic neuropathic facial pain following…

Atypical Facial Pain

2003

Publisher Summary This chapter discusses “atypical facial pain,” which was introduced originally to distinguish trigeminal neuralgia from other facial pain syndromes. The term “atypical facial pain” is also used mostly as a residual category for otherwise unclassifiable pain syndromes in the facial region. The International Headache Society defines atypical facial pain as a persistent facial pain that does not have the characteristics of the cranial neuralgias and is not associated with physical signs or demonstrable organic causes. It is present daily and persists for most or all of the day. It is confined at onset to a limited area on one side of the face and may spread to the upper and l…

Sunct syndrome. Report of a case and treatment update

2015

Short-lasting unilateral neuralgiform headache attacks with conjuntival injection and tearing (SUNCT) is considered a rare trigeminal autonomic cephalgias, a group of primary headache disorders characterized by brief episodes of severe unilateral headache in the distribution territory of the trigeminal nerve, accompanied by prominent ipsilateral and cranial parasympathetic autonomic features. The present report describes a SUNCT syndrome in a 64-year-old male who had been diagnosed with trigeminal neuralgia several years ago. The patient reported stabbing pain in the orbital zone and in the left upper maxillary region, of great intensity, brief duration, and a frequency of 20-100 attacks a …
