Fibromyalgia (FM) is a chronic pain syndrome that affects at least 2% of the adult population. It is characterised by widespread pain, fatigue, sleep alterations and distress, and emerging evidence suggests a central nervous system (CNS) malfunction that increases pain transmission and perception. FM is often associated with other diseases that act as confounding and aggravating factors, such as rheumatoid arthritis (RA), spondyloarthritides (SpA), osteoarthritis (OA) and thyroid disease. Mechanism-based FM management should consider both peripheral and central pain, including effects due to cerebral input and that come from the descending inhibitory pathways. Detection of liver metastases in cancer patients with geographic fatty infiltration of the liver: the added value of contrast-enhanced sonography

Purpose The aim of this study is to assess the role of contrast-enhanced ultrasonography (CEUS) in the detection of liver metastases in cancer patients with geographic liver fatty deposition on greyscale ultrasonography (US). Methods Thirty-seven consecutive cancer patients (24 women and 13 men; age, 33 to 80 years; mean, 58.1 years) with geographic liver fatty deposition, but without any detectable focal liver lesion on greyscale US, underwent sulphur hexafluoride-enhanced US. Recurrent On-Duty Sleepiness and Alertness Management Strategies in Long-Haul Airline Pilots

Introduction We examined whether long-haul airline pilots without recurrent on-duty sleepiness obtain more prior sleep and use more effective in-flight alertness management strategies than their colleagues with recurrent on-duty sleepiness. Methods There were 51 pilots who flew at least twice from Helsinki to Asia. Of them, 44 flew at least twice back to Helsinki following 1 local night. On-duty sleepiness was measured by the Karolinska Sleepiness Scale (KSS), alertness management strategies by a diary, and sleep by a diary and activity monitor. Pilots who rated KSS ≥ 7 on each, some, or none of the flights were classified as "regularly", "sometimes", and "never" sleepy, respectively. This …

Mental adjustment to cancer and quality of life in breast cancer patients: An exploratory study

This study explores the relationship between adjustment to cancer and quality of life in a sample of 68 newly diagnosed non-metastatic breast cancer patients. Responses were assessed on three occasions following communication of the diagnosis and included the Mental Adjustment to Cancer (MAC) scale, a measure of denial and a Spanish quality of life questionnaire for breast cancer patients. The influence of motivation on selective visual attention in states of high vs. low vigilance is poorly understood. To explore the possible differences in the influence of motivation on behavioral performance and neural activity in high and low vigilance levels, we conducted a prolonged 2 h 20 min flanker task and provided monetary rewards during the 20- to 40- and 100- to 120-min intervals of task performance. Interaction between muscle stiffness and stretch reflex sensitivity after long-term stretch-shortening cycle exercise

The short latency stretch-reflex component (M1) and its interactions with muscle stiffness and with muscle performance were investigated before and after long-term stretch-shortening cycle (SSC) exercise. Dramatic fatigue induced reduction in maximal SSC performance capability, and electromyographic activity was accompanied by a consistent decrease in the M1 reflex component and eccentric peak stiffness of the muscle. It can be suggested, therefore, that the decreased muscle performance is not simply a direct effect of central or peripheral fatigue, but is partly due to impairment of the ability to utilize stiffness-related elastic energy.

Fat mass accumulation compromises bone adaptation to load in finnish women: A cross-sectional study spanning three generations

Body weight and lean mass correlate with bone mass, but the relationship between fat mass and bone remains elusive. The study population consisted of 396 girls and 138 premenopausal mothers and 114 postmenopausal grandmothers of these girls. Body composition and tibial length were assessed using dual-energy X-ray absorptiometry (DXA), and bone traits were determined at the tibia using peripheral quantitative computed tomography (pQCT) in the girls at the ages of 11.2 ± 0.8, 13.2 ± 0.9, and 18.3 ± 1.0 years and in the mothers (44.7 ± 4.1 years) and grandmothers (70.7 ± 6.3 years). Background&Aims: Nonalcoholic fatty liver disease (NAFLD) is the leading cause of chronic liver disease and has a strong heritable component. The aim of this study was to identify new genes involved in NAFLD pathogenesis. Methods: We examined rare variants captured by whole-exome sequencing in individuals with severe fibrosis or hepatocellular carcinoma due to NAFLD (severe NAFLD, n=301) after variant prioritization.  We replicated the results in the UK Biobank and the Liver biopsy cohort (n=2268). Objetivo: valorar el uso precoz de la tomografía computarizada (TC) para el diagnóstico, estadificación y manejo de la diverticulitis aguda. Material y métodos: realizamos un estudio prospectivo de 102 pacientes con el diagnóstico clínico de diverticulitis aguda de colon izquierdo.
