Water and Sodium in Heart Failure: A Spotlight on Congestion.

Despite all available therapies, the rates of hospitalization and death from heart failure (HF) remain unacceptably high. The most common reasons for hospital admission are symptoms related to congestion. During hospitalization, most patients respond well to standard therapy and are discharged with significantly improved symptoms. Post-discharge, many patients receive diligent and frequent follow-up. However, rehospitalization rates remain high. One potential explanation is a persistent failure by clinicians to adequately manage congestion in the outpatient setting. The healthy properties and antioxidant activity of smoked paprika in healthy male non-smoking students (age 20.6 ± 1.7 years), from Universidad de Extremadura (Spain), who consumed (2 g/day, 20 times, over 30 days) smoked (La Vera, Extremadura, Spain) or non-smoked (Novelda, Alicante, Spain) paprika were investigated. Anthropometric and cardiovascular measurements and blood and urine samples were taken from overnight fasted subjects for biochemical, hematological, and immunological measurements. Achalasia with megaesophagus and tracheal compression in a young patient: A case report.

Highlights • This report emphasizes that physicians should be alert and consider airway obstruction and signs of dyspnea as severe and threatening symptoms in extensive cases of achalasia with megaesophagus. • Early surgical treatment provides a therapeutic option to obviate the occurrence of acute respiratory distress and consecutive complications. • In particular, difficulties in intubation prior to surgery must be considered. Due to potential tracheomalacia, the status of “bull frog neck” in achalasia, including severe tracheal compression caused by megaesophagus with concomitant cervical swelling, may also lead to extubation problems and deserves special care in the postoperative period.

ABSTRACT Aim: To compare local control and toxicity in non melanoma skin cancer treated with electrons and Leipzig applicators. Methods: We evaluated 60 patients (pts) with non melanoma skin cancer treated using electrons and Leipzig applicators (microselectron-HDR afterloader). The lesions were: 30 basal cell cancer, 17 squamous cell cancer, 10 basosquamous cell cancer, 1 dermatofibrosarcoma, 1 keloid, 1 histiocytoma fibrosus. Median age was 54 (25-84). Reconstruction of Nasal Alar Defects with Freestyle Facial Artery Perforator Flaps

In 2009, we have described the use of freestyle facial artery perforator flaps for one-stage nose reconstruction. Since then, several articles have reported the use of facial artery perforator flaps for nose reconstruction. The purpose of this article is to provide an update of the published technique after 10 years of experience. Since 2004, 21 patients have been treated with a freestyle facial artery perforator flap for one-stage reconstruction of the nasal ala. Management of Small Bowel Perforation by a Bizarre Foreign Body in a 55-Year-Old Woman

Introduction. Ingestion of foreign bodies including dentures, fishbone, screw, and/or surgical devices can be a cause of morbidity, and it rarely could be fatal. Presentation of Case. We present the first hitherto reported case of mussel shell ingestion, which caused acute abdominal pain in a 55-year-old woman. The shell pierced ileal loops, and it was found in the abdominal cavity. Discussion. The accidental or voluntary ingestion of a foreign body is an uncommon event compared to the other causes of bowel perforation. Questo articolo analizza il mercato del lusso soffermandosi in particolare sulle caratteristiche principali dei prodotti luxury good. Fondamentali sono: l'unicità, il pregio, la qualità eccellente ed il prezzo elevato. Grazie alla legge di Hengel è stato possibile centrare l'indagine sul diverso andamento incrementale dei beni inferiori, normali e di lusso all'aumento della fascia di reddito da parte dei soggetti. Si è posto l'accento su come il mercato del lusso sia quello maggiormente incrementato da parte dei soggetti di fascia alta. Negli ultimi anni a differenza dei beni inferiori e normali, il mercato dei beni di lusso ha subito un incremento del 35,7%.
