In order to assess the value of the addition of fludarabine as a chemo-modulator to a high-dose AraC based salvage regimen for patients with refractory and relapsed acute myeloid leukemia the German AML Cooperative Group initiated a prospective randomized comparison between fludarabine q 12 hours on days 1,2,8, and 9 in addition to the S-HAI regimen, consisting of high-dose AraC q 12 hours on days 1, 2, 8, and 9 and idarubicin on days 3,4,10, and 11, as compared with S-HAI alone. In order to maintain the highest quality of patient care possible, the German S2k guidelines were updated following a systematic literature search and with the participation of all professional societies and associations involved in the management of the disease. Part 2 addresses issues such as proper risk stratification, the various therapeutic approaches, and prevention as well as follow-up of patients with basal cell carcinoma.

To meet this need, the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2016 estimated global, regional, national, and, for selected locations, subnational cause-specific mortality beginning in the year 1980. Here we report an update to that study, making use of newly available data and improved methods.
