Comparison of different hollow fibre haemodialysis module configurations by a CFD multiscale approach
2021
Objectives The study aims to predict 3-D flow and solute concentrations fields both for blood and dialysate and overall performance parameters (such as dialysate pressure drop and clearance) for different hollow-fibre haemodialysis modules. Methods A multiscale approach was used. At small (unit cell)-scale, dialysate flow and mass transfer around straight cylindrical fibres arranged in regular lattices were simulated. At module-scale, hydraulic permeabilities and mass transfer coefficients derived from small-scale simulations were used to define two different porous media representative of blood and dialysate, sharing the same volume and exchanging solute. Simulations involved different mod…
Vitom® 3D in arterio-venous fistulas: Single centre preliminary surgical experience
2021
Background: Arteriovenous fistula (AVF) is the vascular access of choice for hemodialysis patients. A correct microsurgical vascular anastomoses is essential to the optimal care of patients. Loupes can help surgeons perform such procedures, but often cause neck or back pain and fatigue. Aim: This study aimed to evaluate whether a video telescopic operating microscope (VITOM®; Karl Storz Endoscopy GmbH, Tuttlingen, Germany) could be used as a substitute for loupes to create microsurgical vascular anastomoses. Methods: We evaluated microsurgical anastomoses with VITOM® in 10 patients (6 male, 4 female) from January 2019 to December 2019. The created anastomoses were 7 side-to-side, 2 side-to-…

Prone Position Facilitates Creation of Ulnar-Basilic Arteriovenous Fistula
2022
Background: The distal ulnar-basilic arteriovenous fistula (UBAVF) is a rarely used alternative type of vascular access for haemodialysis. The location of the vein on the back aspect of the forearm forces an extremely uncomfortable external rotation of the upper limb during surgery when the patient is in a supine position. Methods: We present a new approach towards creating UBAVF, which involves placing the patient in the prone position, thus eliminating the aforementioned inconvenience. The procedure was performed and described in a 46-year-old patient with chronic kidney disease (CKD) due to diabetic nephropathy. In the period from September 2021 to December 2021, we created an additional…
