Pattern of drug use by advanced cancer patients followed at home

The aim of this study was to document the drugs most commonly prescribed to control symptoms in advanced cancer patients being followed at home. We analyzed data for 128 patients admitted to a home palliative care program from January 1993 to January 1995. All patients were followed at home until death by a team consisting of doctors and nurses, and were given two or three medical examinations a week. The most frequently prescribed drugs were analgesics and drugs commonly used to prevent NSAID-induced gastric toxicity. Slow-release morphine was the analgesic used most often. The early 1990s economic setback brought significant reforms favoring the outsourcing of care in Finnish municipalities. Here, outsourcing refers to the practice of municipalities employing private organizations through different means (e.g., open tendering) to deliver public care services. In this context, this study examines the growth in the outsourcing of service housing and home-help services in 311 municipalities from 2001 to 2015 and investigates the municipal factors associated with outsourcing using four dimensions: care needs, population size, economic situation, and political ideology of the municipality. The purpose of this study was to clarify the extent to which elderly people have difficulties in mobility, and determine their association with socioeconomic factors, dwelling environment and use of services. The study groups were composed of two random samples of 800 persons aged 65–74 and 75–84, respectively. In all, 1224 non- institutionalized persons (80%) were interviewed at home. The respondents were asked to assess their ability to get about the house, negotiate stairs and walk outdoors, as well as manage certain physical exercise tasks. It is not known whether exercise alleviates depressive symptoms in frail, very old people recuperating from an acute illness.
