Hypoplasia of the posterior mitral valve leaflet (PMVL) is a very rare finding in adulthood and can coexist with other congenital heart defects. In this image, a transesophageal echocardiography (TOE) carried out on a 59-year-old woman with a 2-month history of dyspnea revealed a hypoplastic PMVL causing severe mitral regurgitation associated with a secundum-type atrial septal defect (ASD) with left-to-right shunting. This case demonstrates how essential 3-dimensional TOE is for a comprehensive assessment of the mitral valve and to improve the diagnostic accuracy of concomitant congenital heart abnormalities.

Background. Cerebral venous air entrapment is a rare finding on cranial computed tomography (CT) scan. Peripheral air embolism is discussed as a potential cause. However, the mechanism of retrograde passage through internal jugular valves and veins is unclear. Case Report. The case of a patient is reported, who had air entrapment in the left cavernous sinus. A child with basilar artery occlusion in the neonatal period is reported. The occlusion was documented by unenhanced computed tomography performed in the neonatal period demonstrating a “dense” artery at the tip of the basilar artery. The pattern of cerebral damage on MRI scan at 10 years of age confirmed the site of the vascular occlusion. The evidence suggests that embolization was the operating pathogenic mechanism of cerebral vascular occlusion. Abstract Objective: To report a case of premature ovarian failure (POF) associated with an ovarian sex cord tumor with annular tubules. Design: Case report. Setting: Reproductive endocrinology unit in a tertiary academic center. Patient(s): A 20-year-old woman with POF. Intervention(s): Biopsy of the rudimentary ovary by laparoscopy. Main Outcome Measure(s): Protocol for POF investigation and histological study of the ovarian sample. Result(s): An ovarian sex cord tumor with annular tubules was detected in the rudimentary right ovary. Conclusion(s): The rare ovarian sex cord tumor with annular tubules, which may be hormonally active, was detected in a case of POF.

Methods A prospective cohort study in 19,469 adults with diabetes, free of cardiovascular disease, attending primary care in Spain (2008–2011).
