Pacemaker-Based Cardiac Neuromodulation Therapy in Patients With Hypertension: A Pilot Study.

Background In prior unblinded studies, cardiac neuromodulation therapy (CNT) employing a sequence of variably timed short and longer atrioventricular intervals yielded sustained reductions of systolic blood pressure (SBP) in patients with hypertension. The effects of CNT on SBP were investigated in this double‐blind randomized pilot study. Methods and Results Eligible patients had daytime ambulatory SBP (aSBP) ≥130 mm Hg and office SBP ≥140 mm Hg despite taking ≥1 antihypertensive medication, and an indication for a dual‐chamber pacemaker. STUDY PROPOSAL: Proposal to evaluate the response of left ventricular function of 30 patients with challenge hypertension to 50 or 100 mg of canrenone. Evaluation will be performed by repeated control of echocardiographic parameters, left atrial function and NT-proBNP. We also will use Tissue Doppler Imaging to identify and follow early left ventricular dysfunction. Careful monitoring of serum creatinine and electrolyte levels will be carried out. At the end of the study we will be able to identify the best dose of aldosterone we can recommend to control hypertension and left ventricular diastolic dysfunction. Effects of a short-term hypoenergetic diet on morphofunctional left ventricular parameters in centrally obese subjects

BACKGROUND: We aimed to study centrally obese subjects without other diseases, to establish whether a short-term hypoenergetic balanced regimen is able to positively modify left ventricular (LV) patterns. METHODS: We studied 32 obese subjects (out of 52 recruited for this study) with central fat distribution and without associated diseases. Each subject had undergone a moderately hypoenergetic diet for a four-month follow-up period and had a regular loss in weight. Some relevant clinical and echocardiographic parameters were evaluated.
