Management of a crown-root fracture : a novel technique with interdisciplinary approach

Anterior teeth with subgingival fractures require a complex treatment plan that addresses biologic, functional and aesthetic factors. This case report describes the management of a crown-root fractured maxillary left central incisor. An interdisciplinary approach was used to restore the tooth due to the complex nature of the treatment. Orthodontic extrusion was performed to move the fracture line above the alveolar bone and periodontal surgery to recontour the altered gingival margin. Finally, the incisor was restored performing a root canal retreatment with a fiber post and a full ceramic crown. The treatment resulted in secured periodontal health and good aesthetics. In bladder exstrophy primary reconstruction remains the gold standard worldwide. Despite various types of osteotomies the permanent correction of pubic diastasis remains a challenge. In maxillofacial surgery callus distraction is a routine treatment for hypoplastic mandibles. Originally described by Ilizarov, this method provides stable and true bone lengthening after gradual distraction of an osteotomy site as long as the periosteum remains intact. Current debate exists around whether a presumed eccentric exercise, the Nordic hamstring exercise (NHE), actually causes active hamstring muscle lengthening. This is because of the decoupling that can occur between the muscle fascicle and muscle-tendon unit (MTU) length changes in relatively compliant human lower-limb MTUs, which results in MTU lengthening not necessarily causing muscle fascicle lengthening. Evoked responses to transcranial and electrical stimulation during isometric and lengthening contractions of the soleus muscle

The aim of this study was to assess differences in motor control between isometric and lengthening contractions of the soleus muscle. Evoked responses to TMS (MEPs) and electrical stimulation of the peripheral nerve (H-reflexes) where recorded at rest and during isometric and lengthening contractions of the soleus muscle at 20%, 40%, 60% and 80% MVC. Torque and background EMG were averaged over a time window of 100ms prior to stimulation. MEPs were found to be significantly lower (P < 0.05) during lengthening contractions compared to isometric contractions at 40%, 60% and 80% MVC (10.95 ± 0.05 mV vs 9.93 ±0.06 mV ; 1.47 ± 0.07 mV vs 9.7 ± 0.08 mV; 11.48 ± 0.08 mV ± vs 10.14 ± 0.07 mV).
