Background: Some studies have shown that switching patients from one tumor necrosis factor (TNF)-alfa inhibitor to another may be beneficial when they have an inadequate response or an adverse event. Objective: We sought to assess the variables predicting the efficacy of the second TNF-alfa inhibitor in patients discontinuing the first TNF-alfa inhibitor. Methods: Data from all 5423 consecutive patients starting TNF-alfa inhibitor therapy for psoriasis between September 2005 and September 2010 who were included in the Italian Psocare registry were analyzed. Polycitemia vera and pulmonary embolism: case report

Pulmonary embolism is a rare clinical onset of chronic myeloproliferative diseases. Early diagnosis is very important because medical therapy reduces both mortality and morbility. We describe a case of pulmonary embolism as clinical onset of an unknown myeloproliferative disorder. On the basis of our experience is very important early diagnosis and therapy to reduce incidence of later major thrombotic complications.

Background. Physical activity and occupational exposures appeared to play a relevant role in pathogenesis of amyotrophic lateral sclerosis (ALS), a neurodegenerative disease of unknown origin. Materials and methods. We aimed to make an overview of the clinical characteristics and life - style (occupation and sport) of a population of 395 patients with ALS from campania, in southern Italy. Results. ALS onset resulted anticipated of about 11 years in industry workers, whilst the more frequent site of onset among farmers was upper limbs. compared to non-athletes, athletes, particu- larly soccer players, showed a 7 years anticipation of ALS onset, with higher mortality after 5 years. Objective. To evaluate root canal enlargement following mechanical shaping using 2 nickel titanium rotary systems.Material and Methods. Forty single-rooted teeth were immersed in resin and sectioned perpendicular to the long axis at 4, 8, and 12 mm from the apex. Digital capture of sections was performed before and after canal instrumentation using Mtwo and BioRace instruments. The area increase of endodontic space was calculated by subtraction.Results. Background: The aim of this study is to verify the variations of the temperature inside the root canal with three different gutta-percha heating systems. Materials and Methods: A split-tooth model was built with a lateral groove at 3.5-mm from working length (WL) in order to insert a Chromel-Alumel K-type thermocouple placed in contact with gutta-percha. The thermocouple was connected to a digital thermometer. Sixty obturations were made with medium gutta-percha points. Objectives: This study evaluated the in vivo effectiveness of Universal Dentin Sealant (UDS), a new resin-based material, as dentinal desensitizing agent on dentin morphology and clinical symptoms. Materials and methods: Thirty premolars, exhibiting non-carious cervical lesions, and scheduled for extraction for periodontal reasons, were selected for the ultrastructural study.
