Post-synovectomy changes in the articular cartilage

Synovectomy of the left knee was performed in 37 immature rabbits, using the right knee for control. In the first set of experiments the articular cartilage was examined at weekly intervals for 8 weeks following the operation, paying particular attention to the metachromatic changes in the ground substance of the cartilage. In the second set of experiments, restoration of synovium was examined. In the third and fourth set of experiments, the uptake of S35 by the cartilage was assessed using autoradiography and densitometry. In the fifth set of experiments, alterations in S35 uptake by the chondrocitic cells of the cartilage were studied by electron microscopy.
Waterjet dissection in pediatric cranioplasty

Object Waterjet dissection has been shown to separate tissues of different resistance, with preservation of blood vessels. In cranioplasty, separation of subcutaneous tissue and dura mater is often difficult to achieve because the various tissue layers strongly adhere to each other after decompressive craniotomy. In the present study, the potential advantages and drawbacks of the waterjet technique in cranioplasty after craniectomy and duraplasty are addressed. Anterior dislocation in a total knee arthroplasty: A case report and literature review.

Dislocations of a total knee arthroplasty (TKA) are an uncommon injury and only few cases of anterior dislocations have been reported. We report a rare case of anterior dislocation of a 10-year-old posterior stabilized total knee arthroplasty in a 74-year-old woman. The patient was successfully treated by close reduction of the dislocation followed by immobilization in full extension with a long leg cast. Because of a high risk of neurovascular complications, a high index of suspicion for vascular injury must be maintained. For this reason, a proper diagnosis is required and immediate reduction is recommended.

Between 1974 and 1984 16 patients with rheumatoid arthritis, who had elbow synovectomy and excision of the radial head, were revised with an average follow-up of 5 years after operation. Resection of the radial head was performed on 16 elbows, in 3 elbows the ulnar nerve was transposed. The indication for the operation were pain, swelling, warmth and radiological destruction. The parameters used, to assess efficacy of the operation were range of motion, local finding and patient satisfaction. Synovectomy results were good especially in respect of relief of pain and range of motion. The good results justify elbow synovectomy even in the late stages of disease.

Osteoporosis is a noncommunicable disease with increasing incidence at a global level. The burden of the disease particularly concerns women. A drastic and rapid decline in the circulating levels of estrogens, a key regulator of bone metabolism, is responsible for the increase in bone loss after menopause. Cells with a role in bone metabolism, osteoblasts, osteoclasts and osteocytes, have estrogen receptors. The fall in estrogens increases the differentiation of osteoclasts from progenitors in the bone marrow. A significant expansion in the resorption process follows.

Medialization of the center of rotation in reverse shoulder arthroplasty (RSA) results in greater excursion of the proximal humerus especially in adduction leading to a mechanical conflict against the neck of the scapula, thus creating a specific complication: scapular notching, in up to 90 % of RSA. We describe our third generation of RSA in Dijon, the Duocentric prosthesis. This prosthesis provides a new proposition to prevent the notching problem with an inferior overhang integrated to the glenoid baseplate. OBJECTIVE: The present investigation should elucidate which assessment technique for bone quality is most appropriate to estimate preoperatively fixation strength of instrumental spine fusions. METHODS: VDS-screw fixation strength in 50 human cadaveric vertebral bodies was approximated by means of pullout force assessment. Bone quality was assessed by Dual Energy X-ray Absorptiometry (DEXA), Quantitative Computed Tomography (QCT), MRT and histomorphometry. Chondrosarcoma is one of the most common malignant bone tumors in adults. It use to affect upper arm, pelvis and thigh bone. A wide surgical extirpation represent the gold standard to treat this disorder. In fact, radiotherapy and chemotherapy are no useful. Ultrasound has been reported to be a quick, cheap, and effective imaging modality to guide the interventional procedures in the musculoskeletal system. The use of ultrasound results in increased accuracy of needle placement associated with a reduction of complications. In the upper limb, ultrasound-guided procedures are applied to joints and soft tissues around the shoulder, elbow, wrist, and hand. This article reviews the clinical and technical aspects of the most common procedures performed in this anatomic area.
