BACKGROUND Arachnoid cysts of the paranasal sinus are rare. They have not been described yet in the sphenoid sinus. PATIENT AND METHOD Microscopic-endoscopic endonasal surgery of the sphenoid sinus was performed on a 34-year-old female with a history of chronic headaches and a suspected mucocele of the sphenoid sinus in computed tomography (CT) and magnetic resonance imaging (MRI) studies. An extended arachnoid cyst was found in the enlarged sphenoid sinus, which was obliterated with collagen, fibrin glue, and abdominal fat. Porcine Model for Gluteal Artery Perforator Flap: Anatomy and Technique

Although flap anatomy is well studied on cadavers and microsurgical techniques are well practiced on rats, still there are few training models for learning the techniques of perforator flap harvesting. The cadaver has no bloodstream, so accuracy of dissection cannot be evaluated and flap viability cannot be verified. Training on humans carries a high risk of flap damage. A living model for perforator flap harvest is needed to learn the technique before starting with its clinical application.

Endoscope-assisted Craniotomy

We describe a surgical technique to improve visualization of deep brain structures during craniotomy. The technique, developed in a cadaveric model, combines the use of an operating microscope and a solid-rod lens endoscope. Addition of the endoscope during craniotomy allows the operator to visualize structures that otherwise might not have been seen. An approach to the brain stem is demonstrated.

To determine the significance of local excision (LE) of rectal cancer and discuss oncologic results, a 1st Workshop on LE of rectal cancer was held at the Department of General und Abdominal Surgery, Johannes Gutenberg-University Mainz, Germany. The option of broadening the indication for local excision after neoadjuvant radiochemotherapy (nRCT) of rectal cancer was to be assessed. Local excision of "low risk" T 1 carcinomas was rated as oncologically adequate therapy with good functional results and low complication rates.
