Orbital exenteration is a radical surgery reserved for the treatment of locally invasive or potentially life-threatening orbital tumours.1 Complications occur after 20–25% of exenterations and include tissue necrosis (6%) and infection (3–4%).2–4 In the present report, we describe the management of a post-exenteration orbital infection by the use of maggots. An 82-year-old multimorbid man presented with a fist-sized painless tumour of the left orbit (fig 1A). Two hundred and two consecutive patients with dizziness were evaluated using blind neuro-otological testing and examination, blind psychiatric examination, including structured interviews (according to DSM-IV), the Symptom Check-List (SCL 90 R), and the State-Trait Anxiety Inventory (STAI). In 28% of the patients (N=50) dizziness was of organic origin (O group); in 55.3% (N=99) of psychogenic origin (P group) and in 16.8% comorbid psychiatric disorders were found (Mixed group). In 5.3% (N=10) neither organic nor psychiatric results could be found, which could explain the dizziness (Ideopathic group). Intratumorous expression of a 153-kd protein (p153), which contains an acetylcholine receptor-like epitope, is the only tumor marker described to date that significantly associates with thymoma in paraneoplastic myasthenia gravis (MG). Here, we report that p153 is identical to the midsize neurofilament, as verified by immunohistochemistry, immunofluorescence, and western blot analysis. Furthermore, the acetylcholine receptor-like epitope of the midsize neurofilament (NF-M) was identified by peptide epitope mapping. The lymphoepithelial cyst is a dysontogenic benign cyst originated from the epithelial remnant retained inside the lymphoid tissue during the embryogenesis that occurs in the lateral neck region and, less frequently, in the oral cavity and in the parotid gland. The parotid gland evolvement increased significantly after the HIV discovery and it is related to ductal obstruction phenomenon that goes along with a follicular hyperplasia in the periductal parotid lymph nodes. The Man Behind the Eponym: Hans Biberstein and Follicular Hyperplasia Overlying Dermatofibroma

Hans Biberstein first described the basaloid follicular hyperplasia overlying dermatofibromas in 1923 and published his extensive observations on the subject in 1931. Part of Josef Jadassohn's department in Breslau, he was forced to leave German by the National Socialist regime and spent the rest of his career in New York. After a hiatus of 30 years, the dermatopathologic literature once again began addressing his seminal finding but never gave him proper credit. We suggest Biberstein's sign as an appropriate term for basaloid follicular hyperplasia overlying a dermatofibroma and as a small tribute to a pioneer dermatopathologist.
