Cervical cord compression is a sequela of mucopolysaccharidosis VI, a rare lysosomal storage disorder, and has devastating consequences. An international panel of orthopedic surgeons, neurosurgeons, anesthesiologists, neuroradiologists, metabolic pediatricians, and geneticists pooled their clinical expertise to codify recommendations for diagnosing, monitoring, and managing cervical cord compression; for surgical intervention criteria; and for best airway management practices during imaging or anesthesia. Background The persistence of synchondrosis in adulthood can confound diagnostic decisions made during patient management. Case Description A 59-year-old woman who presented neck pain, acute headache, and acute cervical myelopathy symptoms after suffering whiplash grade 3 in a car rear-end impact underwent a conventional radiologic study that revealed no fracture and no anatomic spine variations. W artykule przedstawiono przypadek pacjentki cierpiącej z powodu mielopatii szyjnej wynikającej z nierozpoznanego przez wiele lat skostnienia więzadła podłużnego tylnego (OPLL – ossification of posterior longitudinal ligament) występującego w obrębie szyjnego odcinka kręgosłupa. W pracy, poza przedstawieniem przypadku klinicznego, zaprezentowano także patofizjologię, objawy, metody diagnostyki oraz leczenia tego bardzo rzadkiego w populacji europejskiej schorzenia.
