Primary glioblastomas with and without EGFR amplification: Relationship to genetic alterations and clinicopathological features

2009

Glioblastomas express a notable heterogeneity in both the histological and cell patterns with glial astrocytic differentiation. Primary glioblastoma, which is the most frequent presentation (90-95%), occurs mainly in older patients and arises de novo, without any clinical or histological evidence of a less malignant precursor lesion. EGFR amplification has been identified as a genetic hallmark of primary glioblastomas and occurs in 40-60% of cases. However, there exist primary glioblastomas without EGFR amplification/overexpression. The purpose of this study was to stabilize the association between cases with and without EGFR gene amplification with clinical and genetic parameters in 45 cas…

The multifaceted spectrum of liver cirrhosis in older hospitalised patients: Analysis of the REPOSI registry

2021

Abstract Background Knowledge on the main clinical and prognostic characteristics of older multimorbid subjects with liver cirrhosis (LC) admitted to acute medical wards is scarce. Objectives To estimate the prevalence of LC among older patients admitted to acute medical wards and to assess the main clinical characteristics of LC along with its association with major clinical outcomes and to explore the possibility that well-distinguished phenotypic profiles of LC have classificatory and prognostic properties. Methods A cohort of 6,193 older subjects hospitalised between 2010 and 2018 and included in the REPOSI registry was analysed. Results LC was diagnosed in 315 patients (5%). LC was ass…

Voice and choice for users and carers? Developments in patterns of care for older people in Australia, England and Finland

2012

This article identifies key trends over the last 20 years in residential and community care for older people in England, Finland and Australia, investigating the extent of ‘de-institutionalisation’, ‘privatisation’ and ‘individualisation’. The concepts of collective and individual ‘voice’ and ‘choice’ are used to interrogate the roles of collective and individual actors, older people and carers, in influencing policy formulation. While these three processes have been pursued by policy-makers in each country, their implementation is illuminated by understanding how ‘voice’ and ‘choice’ have been operationalised – individually and collectively – in each context. In the reshaping of eldercare…

Screening for Cognitive Impairment in Hospitalized Older Patients

2021

Abstract. This study develops a screening method for older patients based on nursing procedure to predict complications associated with cognitive impairment during a hospital stay. This multicenter diagnostic cohort study among older hospitalized patients was carried out in eight primary-care hospitals. Following admission of the patients, trained nurses conducted a structured interview and the Mini-Cog as gold standard for cognitive status; complications during the hospital stay were also monitored. In a second step, the structured interview combined with the Mini-Cog revealed whether there was any abnormality in the interview, making it possible to validly and reliably as well as sensiti…

Expertise

2017

Several sets of guidelines have been published recently and more are in the works. The very recent American College of Physicians/American Academy of Family Practitioners guidelines were put together by a set of authors and consultants without any expertise in the topic under discussion, that is, hypertension. Although we are not maintaining that all guidelines should be written exclusively by experts, complete lack of expertise among guideline authors is not acceptable. ispartof: Journal Of Hypertension vol:35 issue:8 pages:1564-1566 ispartof: location:Netherlands status: published

Potentially inappropriate prescribing in institutionalised older patients in Spain: the STOPP-START criteria compared with the Beers criteria

2011

Objective: The aims of this study were to identify potentially inappropriate prescribing using the Beers and STOPP criteria. The START criteria were applied to detect prescription omission in the geriatric population. We compared the utility of these criteria in institutionalised older people. Methods: Descriptive study reviewing the medication and clinical records of 81 residents (aged 65 years and more) by pharmacists in a nursing home in the Lleida region (Spain). Results: The mean patients´ age was 84 (SD=8) years, with an average of 5 drugs per resident (total prescriptions: 416 medicines). The Beers criteria identified potentially inappropriate medication use in 25% of patients and 48…

A LEGAL OPINION ON WHETHER A PERSON PERFORMING THE FUNCTION (CHURCH OFFICE) OF THE ECONOMIST OF THE POLISH BISHOPS’ CONFERENCE SATISFIES THE CONDITIO…

2019

Przedmiotem opinii jest udzielenie odpowiedzi na pytanie: czy osoba pełniąca funkcję (urząd kościelny) Ekonoma Konferencji Episkopatu Polski wypełnia przesłankę określoną w przepisie art. 44 ust. 3 ustawy z dnia 28 sierpnia 1997 r. o organizacji i funkcjonowaniu funduszy emerytalnych, czyli należy do kręgu tzw. „osób zależnych”? Przeprowadzona analiza stanu prawnego i faktycznego w tym konkretnym przypadku doprowadziła do konkluzji, że sam fakt bycia Ekonomem KEP nie powoduje, że ziszcza się negatywna przesłanka z art. 44 ust. 3 ustawy o organizacji i funkcjonowaniu funduszy emerytalnych. Ponieważ jednak osoba wykonująca funkcję Ekonoma KEP pozostaje równocześnie w stosunku pracy z jednostk…

Patterns of poverty among elderly Americans: a Latent Class Markov Model

2017

ABSTRACTThis article studies poverty persistence and the role of social security programmes on poverty among elderly in the US. We use a Latent Markov model to disentangle unobserved heterogeneity and state dependence. Because of its dynamic nature, unobserved heterogeneity is modelled to vary over time. This allows to capture different latent states of poverty that change over time. Result indicates the existence of three unobserved types evolving over time according to their propensity to be poor. Moreover, a strong persistence in poverty especially for women, individuals living alone and ethnic minorities is found. Finally, the estimates indicate that giving social assistance tends to re…

Treating cancer in older and oldest old patients.

2015

The so-called “silver tsunami” is a metaphor that the individuals 65 and older represent the most rapidly growing segment of the Western world population. Aging is an ongoing process that leads to the loss of functional reserve of multiple organ systems, increased susceptibility to stress, it is associated with increased prevalence of chronic disease, and functional dependence. Determined by a combination of genetic and environmental factors, this process is highly individualized and poorly reflected in chronologic age. The heterogeneity and the complexity of the older old population represent the main challenge to the treatment of cancer in those patients. We should discern "fit" elderly i…

Appropriateness of oral anticoagulant therapy prescription and its associated factors in hospitalized older people with atrial fibrillation

2018

Aims Although oral anticoagulants (OACs) are effective in preventing stroke in older people with atrial fibrillation (AF), they are often underused in this particularly high-risk population. The aim of the present study was to assess the appropriateness of OAC prescription and its associated factors in hospitalized patients aged 65 years or older. Methods Data were obtained from the retrospective phase of Simulation-based Technologies to Improve the Appropriate Use of Oral Anticoagulants in Hospitalized Elderly Patients With Atrial Fibrillation (SIM-AF) study, held in 32 Italian internal medicine and geriatric wards. The appropriateness of OAC prescription was assessed, grouping patients in…
