Bridging from research evidence to health policy and from health policy to clinical practice: the case of the Spanish strategy for mental health
2009
Introduction: In the last 30 years the Spanish NHS has evolved from a tightly centralized structure to seventeen autonomous regional health systems. As a result a number of benefits have been obtained but also important side-effects such as unjustified variation in clinical practice and a substantial loss of equity. Description: To deal with these unwanted developments the Spanish Inter-territorial Board of the NHS has recently approved and launched several health strategies on priority areas: diabetes, coronary ischemia, palliative care, cancer and mental health. The strategies are based on scientific evidence or consensus and they have been agreed on by all the regions. However, the degre…

All the interviewees lived at home, experiencing different life situations and suffering from different types of cancer. The interviews were recorded and transcribed verbatim. Data analysis was carried out using the descriptive phenomenological method presen…
Early palliative care of non‑small cell lung cancer in the context of immunotherapy (Review)
2020
Abstract. The most common cause of mortality due to malig‑ nant neoplasms in the general population around the world is lung cancer. In the last 10 years, there has been an enormous improvement in the treatment of this disease, mainly due to the immunotherapy that activates the immune system to fight cancer. Patients with metastatic non‑small cell lung cancer are a special group of patients requiring not only cancer treatment but also considerable support in the treatment of cancer‑related problems, as well as comorbidities. Early palliative care is important in this area. In addition, there is certain evidence that medicines most commonly administered in palliative care may lower the effic…
Comprehensive geriatric assessment is an essential tool to support treatment decisions in elderly patients with diffuse large B-cell lymphoma: A pros…
2015
We performed a multicenter study to validate the concept that a simple comprehensive geriatric assessment (CGA) can identify elderly, non-fit patients with diffuse large B-cell lymphoma (DLBCL) in whom curative treatment is not better then palliation, and to analyze potential benefits of treatment modulation after further subdividing the non-fit category by CGA criteria. One hundred and seventy-three patients aged > 69 treated with curative or palliative intent by clinical judgement only were grouped according to CGA into fit (46%), unfit (16%) and frail (38%) categories. Two-year overall survival (OS) was significantly better in fit than in non-fit patients (84% vs. 47%; p < 0.0001).…
Cholangitis prevention in endoscopic Klatskin tumor palliation: air cholangiography technique.
2009
INTRODUCTION: Endoscopic biliary drainage is the treatment of choice for inoperable hilar cholangiocarcinoma (so-called Klatskin tumor). Cholangitis is the main complication post-endoscopic retrograde cholangiopancreatography (ERCP) in Klatskin patients, specially when medium contrast is injected into biliary tree that could not be subsequently drained. Bacterial cholangitis is the principal cause of mortality in these patients. The aim of this study is to analyze cholangitis rate resulting from the use of air versus iodine contrast to obtain cholangiography during ERCP. METHODS: In 9 years, 188 inoperable Klatskin patients were recruited and divided into two groups: iodine (A) or air (B) c…

Use of opioid analgesics in the treatment of cancer pain: evidence-based recommendations from the EAPC
2012
Here we provide the updated version of the guidelines of the European Association for Palliative Care (EAPC) on the use of opioids for the treatment of cancer pain. The update was undertaken by the European Palliative Care Research Collaborative. Previous EAPC guidelines were reviewed and compared with other currently available guidelines, and consensus recommendations were created by formal international expert panel. The content of the guidelines was defined according to several topics, each of which was assigned to collaborators who developed systematic literature reviews with a common methodology. The recommendations were developed by a writing committee that combined the evidence deriv…
