Modified cryopreservation and xenotransplantation of human parathyroid tissue.

Introduction: A modified cryopreservation technique for human parathyroid tissue was compared with the standard method using a programmed freezer. Methods: Total parathyroidectomy was performed in three groups of 6-week-old Rowett nude rats. Group I (control) underwent no transplantation of parathyroid tissue (n=9). After 10 days, the rats of groups II (n=15) and III (n=15) underwent xenotransplantation of 20 mg cryopreserved human parathyroid tissue, which had been stored in liquid nitrogen at –196°C for 1–22 months prior to xenotransplantation. The endocrine system is frequently altered after a major burn trauma. Besides the endocrine response to stress characterized by hypercortisolism, several hypothalamus-hypophysis-target gland axes are rapidly perturbed within a few days. These alterations can persist in the long term and deserve an appropriate treatment. Disturbances in water clearance and glucidic metabolism are also common and need to be diagnosed and corrected to decrease morbidity in such patients. Hypocalcemia is a common disorder in the elderly, affecting 10% of the elderly population as a whole, 18% of those in hospital and 36% of those in long-term care [1, 2]. Many conditions can lead to hypocalcemia in adults young and old, e.g. primary hypoparathyroidism, chronic renal failure, gastrointestinal diseases (such as celiac disease or Crohn’s disease), drugs (particularly diuretics, bisphosphonates and antiepileptics), acute pancreatitis and thyroid surgery. AIM OF THE STUDY The surgical success rate and clinical outcome after surgery for primary hyperparathyroidism were evaluated in a prospective long-term follow-up study. PATIENTS AND METHODS 407 patients, 396 with the first manifestation, 6 with recurrent and 5 with persistent disease underwent operation from August 1, 1987 to August 15, 1999. All patients were prospectively investigated in a long-term follow-up study and underwent reexaminations at regular surveillance intervals. The postoperative course is known in 93.9% of all patients. This is a report on a neonatal primary hyperparathyroidism (NPHPT) treated by total parathyroidectomy and autotransplantation of parathyroid tissue into the forearm musculature. Histologically, the glands were composed of a "chief cell hyperplasia" of the parathyroid tissue. Clinical manifestation in neonates and differential diagnosis of HPT are discussed. Indication for operation and surgical procedure are described.
